Licensing Program Analyst (LPA) Cynthia Reyes conducted an unannounced case management Inspection to ensure the health and safety standards as required by the regulations governing child care centers are met. LPA met with Assistant Director Arpine Patatanyan. Director Lianna Navarardyan
arrived shortly after.

Todays inspection was to interview children regarding a previous incident. During LPAs tour of the facility, staff names. positions and children census was taken. The facility was observed to be over capacity. The facility is licensed for 58 preschool children only and LPA observed and counted a total of 66 children in care on this date. LPA observed:

Red class room: 18 children with 2 staff
Yellow class room: 13 children with 2 staff
Green class room: 11 children with 2 staff
Orange class room: 24 children with 2 staff. (This class room was outside on the play ground with 22 orange class children plus 2 more children from the green class room).

LPA observed 1 teacher in the graduation/orange class room attending to an injured child of the orange class and when LPA walked outside LPA observed 1 teacher alone with 23 children. The teacher that was inside with the injured child was suppose to be outside with the other one staff and the other 23 children. A few minutes after the Assistant Director Arpine came outside and LPA informed her of the teacher child ratio and the staff being alone with the 23 children. Arpine stated the teacher came inside to clean a child scrap and she was coming outside. LPA informed her that LPA observed the teacher alone with the 23 children and the staff should have called for help and waited for someone to come before going inside and leaving the one staff alone with the 23 children. LPA also informed the director that she could have taken the child inside to help with the child's injury while the 2 staff stayed outside with the 23 children.

Limitation on Capacity and Ambulatory Status: A licensee shall not operate a child care center beyond the conditions and limitations specified on the license, including the capacity limitation. This requirement is not met as evidenced by: The facility is licensed for 52 preschool and 6 toddlers.

Director stated today is graduation and will ensure after today that they will stay with in their license capacity every day including a special event day. Director shall submit a detailed staff schedule outlining staff room assignments & coverage and procedure for staff covering when a child gets hurt. Director will conduct a training on teacher-child ratios and provide a copy of the sign-in sheet and training agenda for the staff by the POC due date of 08/09/2024.

The following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code of Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health & safe.

Two (2) Type A - A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent Licensee/Director was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit.

A notice of site visit was given and must remain posted for 30 days.
