25 | Licensing Program Analysts (LPAs) Nolan Tcheng and Staicy Perry conducted an unannounced Case Management-Deficiencies inspection for the purpose of addressing deficiencies at the facility. Upon arrival at 3pm, LPAs met with Lead Teacher Deemathi Weerasinghe, to whom the purpose of the inspection was explained. There were children present during the inspection.
Census was taken. There were 22 children with 2 staff members.
During the tour of the facility, LPAs reviewed the back outdoor playground. While walking through the play area at 1:54pm, LPAs observed a child by themselves with no adults near by supervising. Child was sitting in a play car in the play area. Child did not speak to LPAs when asked if anyone was outside with them. LPAs informed Staff #2 of the child outside by themselves. Per Staff #2, "The child was only out there a couple minutes, they must have ran out." Doors were locked and the top lock is not reachable by children in care. Most of the children were asleep in the Building 2 as it was nap time. Staff #1 stated "Both classes went down for nap at 1pm." It is unknown how long the child was outside by themselves. This is a violation of the supervision and the personal rights of children in care.
The deficiencies listed on the following pages were observed by the LPA and are being cited in accordance with California Code of Regulations Title 22. Please see attached LIC 809D for deficiencies that are being cited and need to be cleared to protect the children’s health & safety.
Upon receipt of this report, the Licensee shall post the Notice of Site visit and any licensing report documenting a type “A” deficiency. The report and the Notice of Site visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. REPORT CONTINUES PAGE 1 of 2
