32 | At 9:15 am, the applicants began touring LPA’s through the home starting with the entry way which lead directly to the living room, The living room was inspected and electrical outlets were observed to be covered. LPA observed children toys, tables and chairs in the living room. Next LPA toured the kitchen and observed a child proof safety gate was installed to the kitchen to make it inaccessible to the children. Next LPA toured the one bedroom and observed an individual bed in the bedroom and napping cots. Next LPA toured the bathroom and did not observe any hazards. Per applicants, there is no poisons in the home. LPA observed the required 2A 10BC fire extinguisher was anchored to the wall bathroom. It was purchased on 03/07/2022 as read by LPA from the receipt. Applicant tested the carbon monoxide and smoke detectors in the living room. It sounded off the alarms and heard by LPA to be functional. Detergents, cleaning compounds, medications, and other items which could pose a danger to children were observed to be inaccessible to children.
Next LPA toured the backyard fenced. Per applicants, children will use the back yard for outdoor play time. The outdoor play area was observed to be fenced. LPA observed that the outdoor yard has toys and other materials for children to play with. LPA observe a trampoline in the backyard. The applicant states that supervision is always provided.
The applicants state that they will provide food for children in care. Applicants were advised that if food is brought from the children’s homes, all containers must be labeled with child’s name and properly stored or refrigerated. Per applicants, at 10:33 a.m., there are no pets, weapons, firearms or bodies of water on the premises. There are toys available for children. There is not telephone service available in the home during this inspection. LPA advised applicants that if a child shows signs of illness he/she/they shall be separated from other children. Per applicants, Hayk Rizer smokes in the home, but not inside the home. There are first aid kit in the home, in the living room.
Applicant, Hayk Rizer has proof of CPR and First Aid training as indicated on the certificate. The applicant, Hayk Rizer has proof of Health and Safety training (completion date: 08/11/2021), Pediatric First Aid and CPR (ex. 07/2023). The applicant, Hayk Rizer has proof of immunization against influenza, pertussis, and measles. Applicant, Armine Rshtuni Rizer, does not have 16 hours Pediatric First Aid and CPR and Preventative Health and Safety training certificate, Mandated Reporter training certificate and Proof of immunization against MMR, TDAP, and influenza and TB test clearance on file.
Applicant, Hayk Rizer completed required mandated reporter training on 06/10/2022. Mandated reporter training must be completed every 2 years. www.mandatedreporterca.com
