25 | PRELICENSING INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an announced pre-licensing follow up inspection to the above facility on 11/01/2022. LPA arrived at the facility at 8:40 AM and met with Hayk Rizer and Armine Martirosyan, Applicants who guided analyst on a tour of the facility. Per applicant operation hours will be Monday to Friday, 8:00 a.m. to 10:00 p.m.. Applicants states they will care for children 3 months old to 13 years old.
All areas identified on the facility sketch were inspected. This is a one story home located on the first level.
The home consists of living room, one bedroom, one bathroom, kitchen and backyard. There is a main house in the front that applicants' family live there at 919 E. Elk Ave., Glendale, CA 91205. There is a detached garage and detached one room in the backyard. Per applicants, parents will enter the facility through the side gate which leads to the side yard and daycare area. There is a door in the living room which leads to the main home and per applicants it will be kept closed all the times.
Areas that are accessible to children are as follows: Bathroom in the living room, living room, one bedroom and backyard (fenced).
Areas off limits based on facility sketch submitted to children and parents include: Kitchen, detached room in the backyard, and detached garage.
