25 | CASE- Management INSPECTION CONDUCTED IN ARMENIAN Licensing Program Analyst (LPA) Anomeh Eivazian conducted an unannounced case- management inspection to the above facility on 01/26/24 due to licensee is requesting capacity increase. LPA arrived at the facility at 11:50 AM and met with Meri Mihranian, Licensee who guided analyst on a tour of the facility. A COVID 19 risk assessment was conducted prior to entering the facility. During this inspection individuals who reside in the home were discussed and notes on Confidential Name List (LIC811) and attached to this report. Per licensee, Meri Mihranian facility hours are Monday to Sunday from 12:00 a.m. to 11:55 p.m..
For Large Family Child Care Home Fire Clearance was granted on 01/24/24.
This is a one-story home located on the first level. The home consists of 2 bedrooms, storage in the bedroom, 1 & 1/2 restrooms, living room, dining room, kitchen, laundry room, front yard (fenced) and side yard. There is a house in the backyard with 1067 1/2 Elm Avenue address, which has separated entrance and exit. Per licensee, parents enter the home through the main entrance door which leads to the living room (daycare) area.
Per licensee the children use the half bathroom in the bedroom, one bedroom, living room, dining room, and front yard (fenced). Per licensee areas off limits to children and parents include: One bedroom, one bathroom in the hallway, storage in the bedroom, kitchen, laundry room, and side yard. LPA did not observe stairs in the home.
All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. At 12:10 pm, the licensee began touring LPA’s through the home starting with the entry way which lead directly to the living room (daycare area).
