An announced pre-licensing site inspection was conducted today by Licensing Program Analysts (LPAs) Crystal Green and Mary Silva to inspect and measure facility for capacity determination. Licensee is requesting a change of location for their existing infant program (198000610) from 1401 N. Mission Rd., Los Angeles, 90033. Licensing staff met with Executive Director, Lisa Wilkin and Site Director, Maria Francesca.

This facility is located adjacent to LAC + USC Medical Center. At 10: 15am, Licensing staff was guided through a tour of the proposed infant classroom. The Infant classroom was observed to have its own entrance. Per Director, parents will arrive through the main office and walked directly to the infant classroom to sign in. Prior to entrance into the classroom, staff or parents will be required to wear bootie shoe covers. LPA observed age-appropriate toys and furniture. Play equipment and materials for infants were inspected for cleanliness and safety. Individual storage space was observed to be available for each child. The classroom also has feeding chairs available. The infant napping area was observed located within the classroom. LPA observed the napping area to contain (8) cribs and (20) cots. Per Director, crib sheets and blankets will be washed daily by the hospital.

LPAs observed a changing table within arms reach of a sink. Per Director, children enrolled in the infant program will not be potty trained. There is a fully equipped first aid kit and a fire extinguisher available with the last service date of 10/10/2022. There is smoke detectors and a carbon monoxide detector on site that meets statutory requirements. The isolation area for an ill child will be located in the main office. Ill children will utilize the staff restroom.

LPAs were guided to the outdoor play area. The outdoor area was observed located within the building with open frame and sunlight windows located in the ceiling. All areas around play equipment and slides are cushioned with material to absorb a fall. Drinking water will be provided via a water pitcher with deposiable cups and sippy cups. Page 1 of 3

Facility will prepare snacks and lunches on site for the children in care. There is a kitchen area available next to infant classroom with a refrigerator for storing infant formula, bottles, infant food. Per Licensee, the staff will utilize a crockpot as the bottle warmer.

No transportation will be conducted by the facility.

Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

During Inspection measures were obtained to determine capacity. An approved Fire Clearance has been received by the Department. Based on measurements obtained today, capacity for indoor activity space measured for 18 children and the outdoor space measured 11.

Licensee submitted a waiver request along with their application documents requesting to utilize the outdoor space on a staggered play schedule in order to obtain the capacity request. LPA informed licensee that waivers are not automatically granted and will need to be review by the Regional Office for final approval. LPA obtained pictures of the outdoor play areas during this inspection.

The following corrections will need to be made prior to submitting application for final approval. Correction will be due by 5/15/2023.

• Applicant will need to install a changing table in the staff restroom to accommodate ill infant in care.
