On 06/04/2024 at 1:30 pm, Licensing Program Analyst (LPA), Crystal Green conducted an announced Pre-Licensing inspection for a new license. Upon arrival, LPA met with Applicant Representative Carolina De Villa-Crelencia. Applicant, Arrow Montessori School of San Dimas, Inc., has requested to be licensed for 24 school age children from ages 5 to 14 years old in classroom D located on the second floor of the building. Hours of operation will be Monday to Friday from 6 AM to 6 PM.

This facility is in a two-story building. There is a private school that is registered to through the CA Department of Education that serves Kindergarten till 8th grade that operates on the second floor of the building that is physically separate from the proposed school-age program. There is also a licensed infant program (198016236) and preschool program (198016235) located on the same premise that operators out of the first floor only.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed Carolina De Villa-Crelencia that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. A fully equipped first aid kit is located in the Directors office and upstairs classrooms. There is an operational carbon monoxide detector on site located throughout the premises. All required licensing documents were observed posted in the hallway near the main entrance. Children will be signed in and out near the main entrance of the facility.

LPA continued to tour the facility and measured all indoor and outdoor activity space. This school-age program will operate from a total of one classroom located on the second floor. Total indoor activity space measured 943.00 square footage, which is sufficient to accommodate the requested capacity of 24 Children. LPA observed all indoor activity space to be complete with safe age-appropriate furniture and equipment, including tables, chairs, cubbies, bookshelves, and other activity supplies for the children.

Drinking water is available in the classroom via water fountain attached to the sink located within the classroom. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 05/22/2024 by the Los Angeles Fire Department. LPA observed a total of 2 sinks and 2 toilets available for children’s use. These are sufficient to accommodate the requested capacity of 24 children. Restrooms afford privacy for children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be located in the main office.

There is a fully functioning kitchen located on the premises which will be used to prepare food on site. The kitchen is located on the first floor which will be maintained inaccessible to the children via door that only staff will be allowed to have access to. The kitchen area currently includes refrigerator, sink, and stove, microwave, food prep countertop. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials.

This facility has 3 fully fenced playground areas that are designated for Private School, Preschool Program, Infant Program. Shade is provided via fixed shade structure. LPA observed a fully fenced inground swimming pool located adjacent to the outdoor space designated for the private school. Three side of the fencing was observed made of wrought iron gate 5’8’ inches tall and the fourth side is made of center block which is 5'5 inches tall. The fencing is 2 inches from the ground and the space in between the bars are less than 4 inches wide. There is one singular entrance into the pool area. Per Applicant Representative, the pool entrance is keep locked when not in use. When in use there is a certified lifeguard that is on duty within the pool area. LPA observed a lifeguard present during this inspection. The entrance gate swings away from the pool and has a self-latching gate with a lock. LPA observed that the fencing requirements meets Title 22 regulations at this time.

At this time the school age program does not have its own designated outdoor space. Per Applicant Representative, the school age program will share the outdoor space with the private school using a staggered play schedule. LPA measured the outdoor space currently designated for the private school, which measured for 4052.16 square feet and can accommodate the requested capacity for the school age children.

There are age-appropriate play equipment and a basketball court available on playground area. There is a climbing structure located for children ages 2 to 6 which is properly anchored. A playground waiver to share the play yard at different times in the PM due to the school-age program not having a separate designated outdoor space shall be submitted and approved prior to licensure. Drinking water will be available via personal bottled water. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. Transportation will be provided to the school- age children.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Per Applicant Representative, this facility was built after 2010 and meets the lead test requirements.

Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communications.

No corrections are needed. A license for 24 school-age children will be granted upon a final file review and approval of the waiver request to share usage of the outdoor space using a staggered schedule.
