On 11/12/2024 at 12:00 pm, Licensing Program Analyst (LPA), Crystal Green conducted an announced Pre-Licensing inspection for a Change of Ownership. Upon arrival, LPA met with Applicant Representatives Marine Arebajyan, Taleen Sarian, and Sabrina Sarian. The facility is currently licensed as Infant (198019148) from ages 6 to 18 months in the infant classroom and Preschool with toddler program (198011085) with a capacity of 67 children: 12 toddlers from ages 18 to 24 months and 55 preschoolers from ages 24 months until first grade.

Applicant, HMH Trans. Inc., requested to be licensed for 64 children: 11 infants from ages 3 months to 18 months in the Infant Room, 14 toddlers from ages 18 to 32 months in the Toddler Room 1 and 2, and 39 preschoolers from 32 months to 5 years old in preschool rooms 1, 2, and 3. Hours of operation will be Monday to Friday from 7:30 am to 5:30 pm.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed applicant representatives that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, applicant representatives stated that they will be stored in a secured box located in the kitchen area. A fully equipped first aid kit is also located throughout the facility. There is an operational carbon monoxide detector on site located in the main classroom. All required licensing documents were observed posted near the entrance to the facility. Preschool and Toddler children will be signed in and out at their designated entrance with the ProCare App. Infant children will be signed in and out their designated entrance using paper sign-in sheets.

LPA continued to tour the facility and measured all indoor and outdoor activity space. The total infant indoor activity space measured was 407.77 square footage, which can accommodate 11 infant children. The total toddler indoor activity space measured was 551.14 square footage, which can accommodate 14 toddler children. The total preschool indoor activity space measured was 1374.55 square footage, which can accommodate 39 preschool children.

LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, bookshelves, and other activity supplies for the children. Drinking water will be available to children via a filtered water jugs with children personal water bottle. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 10/31/2024.

Highchairs and feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. Highchairs are made of washable, moisture-resistant material. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. Cribs meet regulatory requirements. LPA observed 6 cribs and 2 playpens available within the Infant classroom. Crib area is separated from activity space by being located in a separate side room with a portable vinyl divider. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs are kept in the infant classroom. Needs and Services Plans and Sleep Plans are kept in the infant classroom.

LPA observed a total of (1) sinks available for infant children’s use. These are sufficient to accommodate the requested capacity of (11) infant children. The toddler program does not have its own separate designated restroom. LPA did observed (1) sink located within the toddler classroom. Per Marine Arebajyan, a waiver will be requested to allow the toddlers to share use of the designated preschool restrooms. LPA observed that the existing licensee has an approved waiver for shared restroom between programs. LPA observed a total of 6 sinks and 5 toilets available for preschool children’s use. These are sufficient to accommodate the requested capacity of children. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the staff restroom.

The facility will provide AM/PM snacks. Lunch is brought into the facility already prepared by a third-party food vendor called Ceibo. There is a kitchen prep area located near the preschool classroom. The kitchen area includes a refrigerator, sink, microwave, and food prep countertop. The kitchen prep area was observed to be free of hazards. The kitchen and food storage areas were observed free of rodents and/or vermin.

The facility currently has a (2) fully fenced playground area: Infant/ Toddler yard and Preschool yard. The fencing surrounding the playground area is made of wrought iron fencing which was observed to be at least four feet high. LPA obtained measurements of the designated area which calculated to be 4,946.36 square footage, which can accommodate the preschool requested capacity. This facility does not have a separate designated outdoor space for the toddler program. Per Marine Arebajyan, a waiver will be requested to allow the toddlers to use the infant yard. LPA observed the exisitng licensee to have an approved waiver for shared infant outdoor space. The Infant/ Toddler yard measured to 830.33 square footage, which can accommodate 11 children at one time. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure.

Shade is provided via fixed shade structure. There are sufficient outdoor age-appropriate toys and play equipment available on the both playground areas. There is adequate cushioning in fall zones of climber provided sand and wood chip pieces. Drinking water will be available via filter water jug with personal water bottles. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant Representative was reminded that any changes to the facility must be reported to and approved by Community Care Licensing. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).
Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA also informed applicant representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. To receive important licensed related information to licensed facilities, visit the CCLD Important Information website at https://www.cdss.ca.gov/inforesources/community-care-licensing/subscribe and select the Child Care option to receive email communications.

During this inspection, LPA obtained a waiver request letter and pictures of the outdoor play area and the restroom area. The Applicant Representative, Marine Arebajyan, was informed that the waiver request will be reviewed by the Regional Office for final approval in order to obtain the capacity of 64 children.

No corrections are needed. Upon a final file review and approval of the outdoor and restroom waiver requests, a license for 64 children will be granted.
