On 03/24/2026, at 12:20 pm, Licensing Program Analyst (LPA) Shushanik Safaryan conducted an unannounced complaint inspection to the above facility for the purpose of delivering the complaint findings. Upon arrival , LPA met Facility Representative Araceli Guzman to whom the purpose of the visit was explained and who guided LPA on tour of the facility. During the inspection, LPA observed 7 infants with 3 staff members, 6 toddlers with 1 staff member and 12 preschoolers with 2 staff members.

Allegations state: Staff inappropriately handled child in care resulting in bruising.Staff do not keep facility in safe condition.

Per Reporting Party(RP) on 12/30/2025, Child#1(C1) returned home with bruising in his back, appearing as two red fingerprints. RP reported that C1 stated they were pinched by Adult1(A1) at the childcare. Pictures of the bruise were provided to LPA for review.

During the investigation, the LPA conducted interviews. According to Staff 1 (S1), A1 was contracted by the facility to provide dance classes in the childcare center. During the interview, C1 disclosed to the LPA that they were pinched by A1 while at the childcare center and were able to demonstrate the location where the pinching occurred. C1's statements were consistent with an information provided to the Reporting Party (RP), LPA, and other staff members S1, S4, and S5.

S1, S4, and S5 reported observing a bruise on C1's back the next day, describing it as two red-purple lines. Staff 2 (S2) stated that they were present in the classroom at the time of the incident ,but did not observe the incident. S2 stated, they heard A1 redirecting C1 two or three times before hearing cry. Staff 3 (S3) stated that they were located on the opposite side of the classroom and heard C1 cry.

The facility provided video footage from the time of the incident. The Licensing Program Analyst (LPA) observed Child 1 (C1) in the group with the other children. However, the LPA was unable to see the incident because of C1's location in the classroom during the incident. During the video footage review, LPA observed Staff 2 (S2) turning their head towards C1, which was confirmed by S2 when C1 cried.

Allegation: Staff do not keep facility in safe condition.

Per Reporting Party (RP) the facility classroom had broken ceiling tiles and water leaks, requiring buckets to collect rainwater. Photographs were provided to the Licensing Program Analyst (LPA) for review.

During the investigation, the Licensing Program Analyst (LPA) toured the facility and conducted interviews with staff and parents. The director stated that the roof had previously been repaired; however, a leak occurred, necessitating the reopening of the ceiling for additional repairs. An invoice dated January 12, 2025, for roof patching was provided to the LPA.

During the interviews, staff reported that water leaks occurred during recent rainfall, and a bucket was placed in the classroom over the weekend to collect the leaking water. Parents disclosed observing water leaks and a bucket in the classroom during rainy periods.

Additionally, concerns were raised by the parents about the open ceiling area in the classroom,while children in care. While touring the facility, the LPA observed loose ceiling tiles in the bathroom and an open ceiling area in one of the classrooms. Page 2 of 3

Based on LPAs interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12 , Chapter 1 ), are being cited on the attached LIC 9099D

LPA Safaryan informed facility representative Araceli Guzman that this report dated 03/24/2026 document one Type A citation. Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Safaryan informed the facility representative to provide a copy of this licensing report dated 03/24/26 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification

An exit Interview was conducted, copy of this report along with Notice of Site visit, Deficiency page and Appeal Rights were explained and provided to the Facility Representative Araceli Guzman on 03/24/2026..

A notice of site visit was given and must remain posted for 30 days in the visible place.

Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Director stated repairs conducted on weekends or after hours when children are not present. Per Director, ceiling was closed and tiles fixed. Pictures will be emailed to LPA when everything is complete. Director stated, if any future issues with the classrooms , arrangments will be made to keep children safe and Department will be notified.

During the interviews staff and parents confirmed water leaks were observed in the classroom and celling was open in one of the classrooms. LPA observed loose tiles and open celling in the classroom, which poses/posed a potential health, safety or personal rights risk to persons in care.
