Licensee was previously licensed with facility number 198021337 at 815 Coronado Dr. Arcadia, CA 91007

All areas identified on the facility sketch were inspected. This is a one-story house. The home consists of 2 bedrooms, 1 bathroom, living room, dining area (classroom), laundry room, kitchen, detached garage, front yard, and backyard. There is a fireplace in the living room which has been blocked off to prevent access to children.

Areas that are accessible to children are as follows: Living room, bathroom, dining area (classroom), and backyard.

Areas off limits based on facility sketch submitted to children and parents include: two bedrooms, kitchen, detached garage, and front yard.

At 8:15am, LPA began tour of the home. Upon entry into the home, the living room was reviewed. There were age appropriate toys and equipment present. LPA observed a fireplace to be made inaccessible to children in care. Sleeping equipment was observed in the living room closet. Cots are being provided for children to nap. First aid supplies were present and accessible.

At 8:25am, LPA observed a 2A10BC fire extinguisher in the home. The service tag showed it was serviced on 05/15/2024. It was anchored to the wall of the living room.

The dining area was reviewed next. It is currently set up as a classroom. There were various age appropriate materials and toys. LPA observed the door to the kitchen to be made inaccessible by door knob cover. LPA advised applicants that a child safety gate may be more efficient in maintaining supervision and safety if adults are grabbing things from the kitchen during operation hours.

The backyard was reviewed for safety. It was observed to be fenced and will be used as the children play area. There was grass throughout and many age appropriate items: bikes, carts, slide, climbing apparatus, and small play house. LPA observed rose bushes next to an AC unit. LPA advised applicants that the accessibility to rose thorns on the bush, and the vents of the AC unit could be dangerous to children in care. Applicants will obtain materials to make both the rose bush and AC unit inaccessible to children.

There is a cat on the premises that will be kept in the off limit area of the house.

Applicant has proof of CPR and First Aid training as indicated on the certificate. The applicant does have proof of Health and Safety training (completion date: 05/13/2023), Pediatric First Aid and CPR (ex. 04/2025). Mandated Reporter training was completed and expires: 5/16/2026 for Alan Tse and 10/30/2026 for Nuoyi Huang.

The following was discussed with the applicant:

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained signed Property Owner/Landlord Consent form (LIC 9149).

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed the safe sleep regulations with the applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed Applicants of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, 11/01/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Per applicant, they do carry liability insurance or a bond in accordance with standard established by Family Child Care statue. Signed statements (LIC282) will be on children’s files if liability insurance is not carried. The law requires Family Child Care provider to carry liability insurance or bond in the amount of $300,000 annually or to maintain the signed statement in the facility file.

Per applicant, there are no dual licenses at this address. Applicant’s email address was obtained during this application process. The applicant was advised that email may be public information.

The applicants both have proof of immunization against influenza, pertussis, and measles.
