1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 12/30/2024 at 1:40 pm, Licensing Program Analyst (LPA), Crystal Green conducted an announced Pre-Licensing inspection for a Change of Location request. Upon arrival, LPA met with Applicant Representative, Viviann Ivory. Applicant, McQufre, Inc., has requested to relocate from their existing licensed Infant and Preschool Programs (191590008 and 191593822) located at 20350 E. Cienega Ave. Covina, CA 91724. The Applicant is requesting to serve 36 children: (12) Infants from ages 6 weeks to 24 months in classroom H2 and (24) Preschoolers from 2 to 5 years old in classroom H1. Hours of operation will be Monday to Friday from 6:30 am to 6:00 pm. This facility was observed to be located on the property of Sunflower Learning Center. There is a separate driveway available to parents for drop off and pick up. LPA informed applicant representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, applicant representative stated that they will be stored in a secured location within the kitchen area. A fully equipped first aid kit is also located in the kitchen area. There is an operational carbon monoxide detector on site located near the kitchen area. All required licensing documents were observed posted near the entrance of each classroom. Children signed in and out will be available at each child designated classroom. LPA continued to tour the facility and measured all indoor and outdoor activity space. The total infant indoor activity space measured was 745.54 square footage, which is sufficient to accommodate the requested overall capacity of 12 children. The total preschool indoor activity space measured was 1139.57 square footage, which is sufficient to accommodate the requested overall capacity of 24 children.

LPA observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots, cribs, bookshelves, and other activity supplies for the children. Infant toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water will be available to children via water bottles into each child’s personal water bottle. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 12/30/2024 by the Los Angeles County Fire Department.

In the infant room, feeding tables have broad-based legs, plastic seats are in good repair and trays lock onto chairs. Changing tables have at least 1” padding covered with moisture-resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. LPA observed 4 cribs available, which meet regulatory requirements. Crib area is separated from activity space via a fixed structure wall partition. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs are kept in the Infant Room. Needs and Services Plans and Sleep Plans are kept in the Infant Room.
LPA observed a total of 2 sinks and 2 toilets available to the preschool children. These are sufficient to accommodate the requested capacity of preschool children. LPA observed 2 sinks and 1 toilet located within the infant classroom. LPA observed the infant toilet to be made inaccessible, per applicant representative children enrolled in the Infant program are not required to be potty trained by the staff. There is a separate staff restroom equipped with a toilet and a sink that is accessible to staff in the main office of Sunflower Learning Center. The isolation area for children who are ill will be located in the Director’s office which is located near the Infant Room. The ill children will use the staff restroom.

The facility will provide AM/PM snacks and lunch will be provided by the parents. There is a kitchen area located on the premises between the Infant classroom and the Preschool classroom. The kitchen area includes a refrigerator, sink, and food prep countertop. The kitchen prep area was observed to be free of hazards. The kitchen and food storage areas were observed free of rodents and/or vermin.

The facility currently has a (2) fully fenced playground areas: Infant Yard and Preschool Yard. The fencing surrounding the playground area is made of wrought iron fencing which was observed to be at least four feet high. LPA obtained measurements of the designated infant outdoor area which calculated to be 2,389.94 square footage, which can accommodate the requested infant capacity.

There are sufficient outdoor age-appropriate toys and play equipment available within playground area. There is adequate cushioning in fall zones of climber provider via rubber outdoor mats. Drinking water will be available via water bottles with each child’s personal water bottles. LPA observed all hazardous items on the playground to be inaccessible to children. Applicant Representatives was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Per Applicant Representative, the children will not use the facility water to provide drinking wThe preschool outdoor area calculated to be 7,700.00 square footage, which can accommodate the requested preschool capacity. There are sufficient outdoor age-appropriate toys and play equipment available within playground area. There is adequate cushioning in fall zones of climber provider via rubber outdoor mats. Drinking water will be available via water bottles with each child’s personal water bottles. LPA observed all hazaater or prepare meals. LPA obtain a copy of Water Bacteriolysis from the applicant. LPA also informed the Applicant Representative of the Lead Testing Requirement.

LPA discussed the safe sleep regulations with applicant representative and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed applicant representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant Representative was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. No corrections are needed. A license for (36) children: (12) Infants from ages 6 weeks to 24 months and (24) Preschoolers from ages 2 to 5 years old will be granted upon a final file review.
