On 2/19/2025 at 10:00 am, Licensing Program Analyst (LPA), Crystal Green conducted an announced Pre-Licensing inspection for an Emergency Relocation due to the recent LA Fire Disaster. The previous facility 198018862 was licensed to operate at 536 E. Mendocino St., Altadena, CA 91001 as a Preschool Program with a capacity of 48 children. Upon arrival, LPA met with Applicant Representative, Laura Cabral. The applicant, Pasadena Waldorf School, has requested to be licensed to serve 38 preschool children from ages 2 to 5 years old. Hours of operation will be Monday to Friday from 9:00 am to 3:00pm.

This facility is located on the premises of Montrose Girls Scouts of Greater Los Angeles. The childcare center will be located on the 2nd floor of the building which has direct access to the streetside. Sign in and out will occur near the entrance of the Preschool classroom. Per Applicant Representative, there will also be a private elementary school that will operate out of the first floor of the building.

All indoor and outdoor activity space utilized for the children was inspected today. LPA informed applicant representative that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, applicant representative stated that they will keep it in a locked medicine box located in the off-limit kitchen area. A fully equipped first aid kit is also kept in the kitchen area. There is an operational carbon monoxide detector on site located within classroom. All required licensing documents were observed posted near the main entrance of the facility.

LPA continued to tour the facility and measured all indoor and outdoor activity space. The total indoor activity space for the preschoolers measured 1397.91 square feet which can accommodate the requested capacity. LPA observed all indoor activity space to be complete with age-appropriate furniture and equipment, including small tables, chairs, cubbies, napping mats, bookshelves, and other activity supplies for the children.

The toys are safe with no sharp edges, splinters, or points, nor made of small parts that can present a choking hazard. Drinking water will be available to children via filter water dispenser machine. LPA observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on 2/19/2025 by LA County Fire Department.

Licensing staff observed a total of 6 sinks, 5 toilets available for children’s use. LPA observed 2 out of 5 toilets to not be age appropriate. Per the Applicant Representative, they will make the toilets age appropriate by obtaining step- stools. Staff will escort children at all times to the restroom. There is a separate staff restroom equipped with a toilet and a sink located on the 1st floor near the front entrance. The ill isolation area will be located near the preschool classroom and will contain a cot.

Facility will provide AM and PM snacks and Lunch will be provided by the parents. LPA observed a full kitchen located within the preschool classroom. The kitchen contained a stove, oven, refrigerator, sink, microwave, and countertop for food prep. The kitchen was observed to be inaccessible to the children via a child safety gate at the entrance.

The facility currently has a fully fenced playground area one which will be designated for the preschool program. Fencing was observed to be a chain linked fence at least four feet high. The total square footage for the designated preschool outdoor activity space is 11,046.20 square feet, which can accommodate the requested capacity. Per Applicant Representative, the private elementary school children will also share the designated preschool outdoor space. LPA discussed the possibility of placing a barrier in the outdoor space to designated separate outdoor space between programs. Per Applicant Representative, they are unable to alter the outdoor space at this time and will submit a waiver requesting to share use of the space using a staggered play schedule. The preschool children will access the outdoor space via the rear entrance of the classroom. LPA observed a platform balcony that leads directly to the outdoor stairwell to the first-floor outdoor space. LPA advised to applicant to obtain a barrier at the top of the stairs to prevent children from traveling down the stairs without supervision. LPA observed a sticky plant located on the first floor outdoors underneath the stairs. Per Applicant Representative, they will make the area underneath the stairs in accessible to the preschool children.

LPA observed the outdoor space to have sufficient shade and outdoor seating. Drinking water is available via portable water jug. LPA observed drinking water faucets located in the outdoor play area. Per Applicant Representative, the child will not have access to the drinking water until the water is tested. Applicant was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

Applicant representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

The following corrections are needed prior to the issuance of the license:
  • The Applicant must obtain and submit proof of a barrier gate placed at the top of the stairwell leading outdoors to the first floor.
  • The Applicant must submit proof of the sticky plant located near the bottom of the stairwells to be made inaccessible to children.
  • The Applicant must submit proof of obtaining step stools and age-appropriate equipment within the designated children’s bathrooms.
  • The Applicant must submit a waiver request for shared use of the outdoor space with the private elementary school.

LPA discussed with the applicant representative the additional pending documents that be requested to complete the application process. Applicant understands that the facility may be issued a provisional license to allow additional time to submit the documentation. Once all corrections are received and reviewed, a provisional license will be issued for (38) preschool children from ages 2 to 5 years will be granted.
