On August 14, 2025 at 1:30 pm Licensing Program Analysts (LPAs) Priscilla Ochoa and Crystal Green conducted an announced Pre-Licensing inspection at the above facility. LPA was greeted by licensee, Felisa Wright, who guided LPAs on a tour of the facility. A church’s grounds keeper and the licensee’s relative were also present during the inspection. The licensee has been affected by the Eaton fire and intends to relocate her Family Child Care to Metropolitan Baptist Church based on the Proclamation, the Governor issued Executive Order N-2-25 on January 8, 2025 (EO N-2-25). The hours of operation will be Monday – Friday from 6 am - 6 pm. The licensee obtained an approved fire clearance for 14 children to use the first floor of the building on 8/05/2025.

At 1:40 pm, the licensee guided LPAs on a tour of facility. The facility is on the grounds as Metropolitan Baptist Church and parents enter through the back doors of building. Per licensee the double doors will remain locked, and parents will ring the door and a staff member will grant them entry. The LPAs walked through the doubled doors and walked down a couple of steps and walked to room 102 which is the daycare classroom. In the room LPAs observed a mounted television, bookshelves, and age-appropriate toys for children in care. LPAs also documented measurements and pictures of the classroom. A changing table was observed to be mounted on the back wall of the classroom and is in arms reach for a portable sink.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

LPAs also observed enough cubbies for children in care to store personal belongings. LPA observed 7 matts and 1 pack n play as napping equipment. Per licensee she has an additional 7 mats, and 3 pack n plays available if needed. Per licensee infants will be napping the in classroom. Based on attendance, the pre-school children will be napping in the cafeteria space and or the classroom. Licensee will remove napping mats and pack n plays when not in use and will be stored appropriately in the hallway next to the classroom on a utility cart. LPAs observed bedding sheets in Ziploc bags for children in care to ensure appropriate storage and cleanliness. Per licensee, the facility will be washing all sheets every Friday for children in care. LPAs also observed a highchair for eating purposes. License then guided LPAs to an additional space that is across the hall from the double doors at the entrance. LPA inspected the cafeteria space and observed tables, chairs, and a television. LPAs also observed electronic cords, devices, and large speakers. Per licensee, the area where the electronics are located will be made inaccessible for children in care. LPAs discussed with license potential use of the space for preschool and school aged children. LPA obtained measurements and pictures of the cafeteria space. LPA also request for the licensee to obtain additional material for preschool and school aged children to use in the cafeteria space. Per licensee, materials are in storage will be assembled and placed in the cafeteria area. LPAs reminded licensee that all children must be under supervision, including visual supervision. All areas were observed to be clean, orderly, comfortable, and well ventilated.

The facility does have a designated girl’s restroom with 3 toilets and 2 handwashing sinks, and the boy’s bathroom obtains 2 toilets, 2 handwashing sinks and 2 urinals. Both bathroom areas are located next to the classroom. All toilets and hand washing facilities are in safe and sanitary operating condition. The licensee will need to purchase step stools for children in care to use the restrooms as toilet bowls are too high for a preschool child to reach. Drinking water is available in the form of personal water bottles that are also taken outside when needed. The facility will provide additional water which will be dispensed from purchased water bottles in classroom for children in care.

LPAs observed stairs that lead to the second floor of the building across from the cafeteria area. LPAs instructed the licensee that the stairway will need to be made inaccessible to children in care. Per licensee, the facility will add a child safety gate at the bottle of the stairs, which will make the second floor inaccessible to children in care. The second floor of the building was also inspected. LPAs obtained measurements and pictures of the classroom that was intended to be used. Per fire clearance, the facility will need to install water sprinklers on the second floor in order to be granted use for the upstairs classroom. Per licensee, the church is in the process of obtaining more information to start the process in installing the water sprinklers. LPAs reminded licensee, if the church makes the necessary corrections to the second floor water sprinklers, she will need to let the department know before the second floor can be in used for child care.

The classroom furniture and equipment were inspected for good repair, free of sharp, loose, or pointed parts. All indoor classrooms were inspected to ensure that the floors have a surface that is safe and clean. All materials and surfaces accessible to children are toxic free.

The facility does have a fully fenced outdoor playground space for children in care. The children will need to cross the parking lot to access the play yard, which is the distance of 168 ft. Per licensee the child will be escorted and supervised to the outdoor play area with one staff member leading the group and a staff member at the back of the group. Staff members will have a clipboard with the sign in/out sheet and emergency contacts. The outdoor space was observed to be covered in grass, obtained picnic tables and basketball court. There is sufficient shade in the form of trees. Per licensee, the facility will purchase an appropriate play structure for ages 2 – 6 years old to use. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards.

LPAs observed the cafeteria area to have a removable patrician area to create a separate isolation area. The children will use the boy’s bathroom as an isolation bathroom. A mat and pack n play will be available if needed.

The facility will provide morning and afternoon snacks for children in care and parents will provide children’s lunch. The licensee will provide LPAs with the snack menu that the facility will follow and it will be posted. The facility will purchase a mini refrigerator that will be placed on the classroom to store additional snacks. There is a kitchen area on site in the cafeteria area. The kitchen area is made inaccessible as the door leading to the kitchen will be locked during daycare hours. LPAs reminded licensee lunch/meals brought from home for children in care must be stored properly, labeled with the child’s name and or refrigerated. All storage containers for solid waste, including moveable bins, have tight fitting covers and are in good repair. The licensee has access to the locked janitorial room which will contain disinfectants, cleaning solutions, poisons and other items that are dangerous to children are stored in an area inaccessible to children. Per licensee there are no weapons, firearms, or bodies of water on the premises.

LPAs observed a functioning carbon monoxide detector in the classroom and a functioning smoke detector in the hallway next to the classroom. LPA observed a fully charged 2A-10BC fire extinguisher. The valve on the fire extinguisher pointed to the fully charged green section. The fire extinguisher was serviced on 10/15/2024.

The facility will have a sign in / sign out sheet for parents to sign in the hallway upon entrance to the building. LPAs informed licensee that the sign in and sign out sheets should document the time the children are dropped off and picked up along with the person who sign in and out the child uses their full legal signature.

LPAs reviewed and discussed children and staff records that are used for a childcare center. LPAs provided licensee the necessary documents for children and staff that are required for a child care center. During the inspection, the licensee provided Early Child Care completed courses transcripts for LPAs to review. Per licensee, she is also currently enrolled in Carrer Bridge program through UCLA extension program to obtain a CTC teachers permit.

Licensee will also submit Early Child Care completed course transcripts for additional staff who will be working with the infants. The licensee will also create an admissions packet for parents to review and sign.

LPAs also reviewed and discussed Child Care Center postings, and licensee will start gathering documents to start posting forms on licensing board. Licensee also provided LPAs a copy of the daily activity schedule. The licensing board will be posted near the entry of the classroom.

Incidental Medical Services (IMS) was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. FACILITY HAS NOT COMPLETED LEAD TESTING: Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Per licensee, the facility will request an attestation letter as the facility will not use water at the facility for drinking nor food preparation. The licensee did state that the facility will test for lead in the near future.

Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The facility will need to make the following corrections, steps stools for the children’s bathroom, copy of sample snack menu, barricade of electrical cords and devices in the cafeteria space, add child safety gate to the bottom of the staircase to make the second floor inaccessible, and obtain additional materials for preschool and school aged children. Once the licensee submits proof of corrections, the information will be reviewed by the Regional Office for final approval.
