Licensing Program Analyst (LPA) Reiko Jones-Modeste met with Applicant Grace Lasisi via TELE_INSPECTION using FACETIME for the purpose of conducting a Pre-Licensing Inspection due to COVID_19 SOE. Applicant is applying for a Preschool License. Also present at the time of visit inspection, Applicants spouse. Applicant is requesting a total capacity of 8 children. No other license is held by Applicant.
All indoor and outdoor areas identified on the facility sketch were toured during this visit. Per Applicant, all visitors will enter through the front gate which leads directly into the parking lot and play yard. LPA observed several required COVID postings at classroom entrance at rear play yard, however NO COVID intake was observed. LPA advised Applicant to place COVID postings and intake at front gate. LIC 311A was issued and Safe Sleep, Lead Poisoning and Car Seat Laws will be sent to Applicant via mail.

LPA observed a changing table near entrance to Classroom #1 adjacent to the children’s restroom near end of hallway. Classroom 1 connected to a small office behind a door with no safety lock, accessible to children in care. Ensuite children’s restroom observed in Classroom #1 with cleaning supplies stored in overhead cabinets. LPA did not observe age-appropriate toys and equipment set up in Classroom #1 or #2 as Applicant stated equipment must be unpacked, delivered and set-up. Some large boxes and trash bags observed.

Some First Aid supplies were observed and inventoried in Classroom #2 however LPA did not observe a thermometer. LPA observed the kit accessible to children in care. Applicant understands that First Aid kits must not be accessible to children in care. Carbon monoxide detectors and smoke detectors were not present as Applicant stated they are to be installed. observed. LPA observed sufficient storage space for children’s belongings including large cubbies in Classroom #1. LPA observed trash cans with closed lids in Classrooms #1 and #2. In Classroom #2 (entrance door located in play yard) some books, toys and equipment was observed in bags and boxes beneath a large table. In addition, LPA observed an un-assembled white board as well as band equipment including drums, bongos and keyboard in a rear corner. Classroom divider(locks with a key) was observed with rows of stacked chairs and storage boxes observed on opposite side. A changing table and bookshelf also observed. Children’s restroom was observed clean and sanitary with three urinals, two toilets and two sinks with no COVID postings.

The facility will not provide lunch only morning and afternoon snacks. Kitchen was observed inaccessible to children in care with cleaning compounds stored beneath the kitchen sink inaccessible to children in care. Kitchen is off-limits behind a locked door. LPA did not observe napping equipment nor sufficient food in appropriate containers in the kitchen area. Applicant stated food, equipment and toys stored in boxes observed in the Classroom #1, #2 and kitchen.



The playground is located at the rear of the parking lot. The surface of the outdoor activity space is not cushioned. No shade or barrier was observed. Some children’s toys and play equipment was observed including a seesaw and an unassembled playhouse. Applicant understands she must provide a barrier between the parking lot and the playground. There is water available indoors however, LPA did not observe water available outdoors. In the side yard off-limits area some hazards observed including stacked tables and propane gas tanks.

Directors Office located next door to Classroom #1 serves as children’s isolation area.

The following requirements are needed prior to the applicant obtaining a preschool license:

1. Barricade around playground in parking lot


2. Appropriate ground cushioning and shade per regulations
3. Isolation area Signage
4. Off-limits areas made inaccessible(adjacent staff bathroom, office, side yard and isolation)
5. Required postings
(COVID, LIC 311A and Personal Rights, Emergency Disaster Plan, Menus, Parents Rights)
6. Appropriate children’s toys and play equipment set-up
7. First Aid kit made inaccessible (missing no-touch thermometer)
8. Carbon Monoxide, Smoke Detectors and Fire Extinguisher
9. Sign in/out sheets
10. Water available in outdoor play yard
Once the above items have been corrected, and all application documents approved, the department will make its determination on whether or not to approve the Applicant for a preschool license.

THE ABOVE CORRECTIONS ARE DUE ON OR BEFORE May 14, 2021.

The Applicant was advised how to access forms and Regulations online at www.ccld.ca.gov. Applicant was made aware that it is his/her responsibility to know the regulations as well as anyone who assists in providing care. The Applicant was advised that inaccessibility of hazards must be constantly reassessed depending on the children in care. Applicant advised that records must be updated with the required documents immediately, and records for all children and staff must be maintained for three (3) years after separation from the facility. Applicant was also advised of the requirement to report Unusual Incidents and/or injuries to the parent/guardian and to Community Care Licensing (CCL) within the time frame specified by the regulation. The LIC 9213, Notification of a Site Visit was issued. This notice is to be posted for 30 consecutive days. Failure to keep this notice posted for the 30 consecutive days will result in an immediate $100 civil penalty. Exit interview conducted with Applicant, and a copy of report was sent via email due to COVID_19 SOE. Appeal rights were issued and discussed.
