Licensing Program Analyss (LPA) Austin Estrada conducted an unannounced required 1 year inspection at the above facility. LPAs met with Director Teliscia Martin-Robinson who guided analyst on a tour of the facility. There were no infants present upon arrival. The facility operates Monday through Friday 7:00AM to 4:00PM. This is a infant program that provides care to infants ages newborn to two years old. There is a preschool program under License # 198400158 and a afterschool program under License # 198400157 also on the premises. All programs are physically separate from one another.
This infant program consists of one classroom. The following was observed during the tour of the facility: LPA observed required posted licensing documents that included, Facility License, LIC 610- Emergency Disaster Plan, Publication (PUB) 393- Notification of Parent Rights, PUB 269- Child Passenger Restraint System, LIC 613A- Notification of Personal Rights. LPAs observed LIC 9148 Earthquake Preparedness, Daily Activity Schedule, and Verification of Disaster and Fire Drills (last conducted on 4/7/23). A facility roster was available.
In the classroom used by infants, furniture and equipment were inspected for age appropriateness and good repair. Toys were observed to be safe, and do not have sharp points, edges, splinters and are not made of small parts that can pose a choking hazard. LPA did not observe any baby walkers or any items that fall into that category present in the facility. The classroom was inspected to ensure that the floors have a surface that is safe and clean. LPA observed carpets and soft play mats on the floor of the classroom. LPA observed cubbies available for children to store their belongings in the classroom. Napping equipment (cribs) were observed to be physically separate from the indoor activity space. LPA observed 6 cribs available in the infant room. All sheets for the cribs are washed twice a week or when needed at the facility. LPA observed a changing table withig an arm’s length from a sink. Per Director water is readily available via bottled water.
Per Director, the facility does not use any water sources at the facility for consumption or food preparation. Parents bring the children’s formula, and their bottles are labeled and stored in the refrigerator if needed. Per Director, the facility provides food to children who eat solid foods. The facility is equipped with a centralized fire alarm system. LPA observed a functioning carbon monoxide detector at the time of inspection.

LPAs observed Fire extinguisher indicated fully charged last serviced on 4/14/23. First Aid kit was observed and inventoried. Disinfectants and cleaning solutions were observed to be inaccessible to children in the classroom.
Per Director, no children currently enrolled are on medication and if medication was stored at the facility, it would be stored in a box in the off limits kitchen area and inaccessible to children in care.
LPA toured the infant outdoor play area, which was observed to have age-appropriate toys and materials for children. The outdoor play area was observed to be free of loose, sharp, and/or pointed parts. LPA observed required cushioning under play structures to absorb a fall. LPA observed infant outdoor play area to be separate from the school age and preschool programs. Per Director, water containers are brought outside for children to drink while outdoors. LPA observed adequate shade in the outdoor area. LPA did not observe any bodies of water at the facility. Per Director staff use a door that is located in the infant room to go to the outdoor play area. Children do not co mingle with preschool or school age children.
Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Menus are posted in each classroom for children's authorized representatives. Menus are available upon request. The facility provides breakfast, lunch and two snacks for children who eat solid food. Per Director food is not made at the facility, it is brought in daily from an outside source. LPA observed the kitchen/ food preparation area, and food storage area to be clean and free of litter, rubbish, rodents, and/or any other vermin. LPA observed the refrigerator to be clean and in operable condition. LPA observed expired food items in the refrigerator and food that was not in its original box was labeled with an expiration date.
Children’s records were reviewed for LIC 613A- Personal Rights, Admission Agreement, Immunization Records, LIC 700- Identification and Emergency Information, LIC 701- Physician’s Report, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 9227- Infant sleep form (0-12 months, Needs and Services Plan, and documentation of 15-minute Infant Sleep Check (0-24 months). All documents were observed to be complete.
Staff records were reviewed for approved Pediatric First Aid and CPR certification, staff qualifications, LIC 508-Criminal Record Statement, LIC 503- Health Screening Report, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, and LIC 9108- Statement of Child and Mandated Reporter Training Certificate. All records were observed to be complete.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Based on LPAs observation and record review no deficiencies will be cited today, 7/20/23.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and Appeal Rights and report were reviewed with the Director.
