32 | LPAs observed Fire extinguisher indicated fully charged last serviced on 4/14/23. First Aid kit was observed and inventoried. Disinfectants and cleaning solutions were observed to be inaccessible to children in the classroom.
Per Director, no children currently enrolled are on medication and if medication was stored at the facility, it would be stored in a box in the off limits kitchen area and inaccessible to children in care.
LPA toured the infant outdoor play area, which was observed to have age-appropriate toys and materials for children. The outdoor play area was observed to be free of loose, sharp, and/or pointed parts. LPA observed required cushioning under play structures to absorb a fall. LPA observed infant outdoor play area to be separate from the school age and preschool programs. Per Director, water containers are brought outside for children to drink while outdoors. LPA observed adequate shade in the outdoor area. LPA did not observe any bodies of water at the facility. Per Director staff use a door that is located in the infant room to go to the outdoor play area. Children do not co mingle with preschool or school age children.
Food and snacks were reviewed for availability, quantity and appropriateness to children in care. Menus are posted in each classroom for children's authorized representatives. Menus are available upon request. The facility provides breakfast, lunch and two snacks for children who eat solid food. Per Director food is not made at the facility, it is brought in daily from an outside source. LPA observed the kitchen/ food preparation area, and food storage area to be clean and free of litter, rubbish, rodents, and/or any other vermin. LPA observed the refrigerator to be clean and in operable condition. LPA observed expired food items in the refrigerator and food that was not in its original box was labeled with an expiration date.
Children’s records were reviewed for LIC 613A- Personal Rights, Admission Agreement, Immunization Records, LIC 700- Identification and Emergency Information, LIC 701- Physician’s Report, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, LIC 9227- Infant sleep form (0-12 months, Needs and Services Plan, and documentation of 15-minute Infant Sleep Check (0-24 months). All documents were observed to be complete.
Staff records were reviewed for approved Pediatric First Aid and CPR certification, staff qualifications, LIC 508-Criminal Record Statement, LIC 503- Health Screening Report, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, and LIC 9108- Statement of Child and Mandated Reporter Training Certificate. All records were observed to be complete.
