25 | Licensing Program Analyst (LPA) Warren Birks conducted an unannounced Case Management inspection. LPA met with Licensee Catalina Bonilla who assisted with the inspection.
The purpose of today's inspection is to amend a previous December 7, 2022 report. LPA informed Licensee the report is amended to reflect the following information left out of the Annual Inspection: Licensee has a small pet dog that is kept inaccessible to children.
Note: LPA also observed the dog being kept in areas that were gated and inaccessible. LPA advised Licensee that if the dog ever has access to children (in the future), to provide extra care and supervision as animals can be unpredictable.
Exit interview was conducted with Licensee Catalina Bonilla. The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site inspection by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. |
