Licensing Program Analysts (LPAs) Alicia Mooberry and Rosaura Valenzuela conducted a Required Annual Inspection on this date. Upon arrival at 10:20am LPAs met with Covena Stewart,Licensee who provided tour of facility. LPA explained the purpose of inspection and provided the inspection Entrance Checklist, LIC 126. LPA inspected rooms/areas on the facility sketch in which child-care services are provided and to which children have access, as well as off limit areas. Per licensee the hours of operation are Monday-Friday 6:30-6:00pm. There were 6 children present during visit, including 2 infant. Also present was Kevin Walker, resident and Zaion Marshall, a volunteer. Per licensee the volunteer is not alone with children and is only present on this day. Individuals residing in the home were discussed and noted.

This is a two-story home which consists of 4 bedrooms, 3 bathrooms, kitchen, dining room, den, living room, backyard, garage, and front yard.

Areas used by children garage (converted to daycare area, bathroom (by garage), living room, dining room, den and backyard. LPA observed that there is a wood burning fireplace in the living room with glass doors observed to be locked. LPA observed no signs of use.

Per Licensee, areas off limits to children and parents include: Entire Upstairs which include 4 bedrooms and 2 restroom upstairs, kitchen and side yard. There is a mesh barrier at the foot of the stairs leading to 2nd floor. The licensee was reminded that as long as there are children age 5 and under, the barrier has to be maintained.

Areas that are used by children were inspected for safety, comfort, cleanliness, telephone service, ventilation and heating. Rooms that are off-limits need to be made inaccessible during operating hours. The licensee does understand that licensing staff may have access to off-limit areas during inspection visit if necessary.

The licensee provides food for children in care.

LPA observed the facility license, Publication (PUB) 394- Notification of Parent Rights and Licensing Form (LIC) 9148- Earthquake Preparedness form posted at the entrance to the home in a location visible to parent/guardians of children in care. LPA observed completed facility records including; The LIC 9040- Facility Roster was not available for review, LPA provided a blank form per licensee request. The LIC 610- Facility Disaster Plan was completed.

Smoke and carbon monoxide detectors were tested and are operable. Fire extinguisher indicated fully charged and was serviced on May 2023, Licensee was reminded that fire extinguisher needs to be serviced yearly. The home maintains telephone service via cell phone. The home is observed to be clean and orderly. There are toys and other age appropriate material available for children. LPA adived licensee to cleaning compounds in area that is inaccessible to children. LPAs reminded the licensee to ensure the bathroom floor is wiped after use.

Licensee states that there are no poisons stored in the home and understands that all poisons must be lock, not only inaccessible to children.

Per Licensee there are no firearms or weapons stored in the home. There are no pools or bodies of water. There are pets in the home including a dog and various pets in tanks: turtle, fish, and tarantula. Licensee was advised to take extra care if children have access to pets in the home.

Isolation area for sick children waiting to be picked up is in living room, away from the other children.

LPA Valenzuela reviewed Safe sleep regulations, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0-24 months, and provided PIN 20-24-CCP.

Licensee states infants sleep in the living room in their own play yard. Staff supervise infant for the duration of the nap and will document 15 minute checks. LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.

LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The outdoor play area backyard was observed to be fenced. LPA observed age appropriate toys and other materials for children to play with including a two level wooded play structure, which is anchored to the ground with cushioned material under structure. Since the family dog used the backyard when not used by children, Licensee was reminded to check the yard prior to children using it to ensure it is safe and sanitary. The Licensee was also reminded to check for wear on the wooden play structure such as lose nails and splintering wood. Facility does not have a pool or similar bodies of water.

Children’s records were reviewed: 6 out of 6 children's record reviewed were missing proof immunization, 2 out of 2 infants were missing 15-minute Infant Sleep Check log (0-24 months). Child #1 (5 month old infant did not have a facility file available.

Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate (has not been renewed since Feb 2019)

LPA discussed Incidental Medical Services – IMS. For IMS information and referenced PIN 22-02-CCP. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on the LPA's observations and records review the following deficiencies will be cited today in accordance with California Title 22 Regulations. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Licensee, Covena Stewart.
