On December 4, 2024 at 3:50pm; Licensing Program Analyst (LPA) Ashley Calderon conducted an unannounced Case Management Visit. LPA was greeted by Licensee Jasmine McDaniel who allowed entry to the facility. LPA disclosed the purpose of today's visit.

LPA Calderon upon arrival in the garage observed at 3:50pm (12) after school children in care in the day care garage with Licensee Jasmine McDaniel. LPA entered home through the garage that leads to the kitchen, Staff #1(S1) was in the kitchen. LPA Calderon alongside with J.McDaniel conducted a tour of the accessible areas to the children in care. LPA observed in the back yard (2) staff assistants, Staff #2 and Staff #3 with (5) infants, deficiency cited. It was observed facility had a census of (17) children present at the facility and is licensed for a capacity of (14) children maximum, deficiency was cited due to facility being over the licensed capacity. LPA reviewed personnel record for facility and at 4:05pm LPA Calderon requested assistance with LPA Dayna Chambers via telephone to get an updated roster search for Staff #1 -Staff #3. At 4:06pm LPA observed after-school children were not in the garage, LPA asked Licensee where did the children go. Licensee stated left with Staff #1 to the park. LPA asked Licensee if Staff #1 left alone with the (12) children per Licensee stated yes, children go to a program at the park and met up with park workers. Per LPA D.Chambers at 4:15pm verified that Staff #1 does not have fingerprint clearance. Per Licensee J.McDaniel Staff #1 started working at the facility on Monday 12/2/24 and Staff #1 stated working here this week. Licensee stated believing Staff #1 was cleared and licensee was unable to provide clearance documentation to LPA Calderon for S1, deficiency cited for Staff #1 who is not cleared. At 4:12pm; cleared Staff #4 arrived at the facility Licensee stated is going to go to the park with Staff #1 and the children and will come back to the facility. At 4:30pm Staff #4 returned from the park with (8) children. Licensee stated Staff #1 was told to go home and that children started to get picked up my parents from the park. LPA did not observe Staff #1 when returning from the park. LPA observed parents starting to pick up children at 3:57pm and on going through LPA presence at the facility. Per California Code of Regulations, Title 22, Division 12, (3) deficiencies and (1) civil penalty (LIC421IM) was cited during today's visit. Citation on LIC809-D pages, (3) Type A's were given on today's visit and a repeated violation was given.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).

The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit.

A Notice of Site Visit was given and must be posted for 30 days and appeal rights were provided.

An exit interview was conducted with Licensee Jasmine McDaniel.

 LPA Calderon provided LIC 9188 and Guardian contact info. Licensee during visit told Staff #1 that they need to leave facility. By 12/31/24 Licensee will update LPA Calderon on Staff #1 clearance status and submit clearance letter by date provided.

It was observed that at 3:57pm children started to get picked up by authorized representatives. Licensee stated will create a Time Slot for children in care and will disenroll children if needed. LPA requested time slot or letter of disenrollment provided to parents to be sent to LPA Calderon by 12/9/24.
