25 | Licensing Program Analyst (LPA) LPA Ashley Calderon arrived to the facility and met with Brandi Bowman who granted LPA entry to the facility. The purpose of the inspection was to conduct a complaint inspection. Due to LPA observation LPA conducted a Case Management inspection. Bowman notified via telephonically Licensee Jasmine McDaniel that LPA was present at the facility.
- At 9:25am LPA Calderon observed: (6) infants present and (1) child and (2) staff assistance. At 9:27am additional (1) child arrived at the facility with a staff assistant (1), making census 8.
- At 9:38am, infant child #1 got picked up from facility with authorized representative.
- At 10:01am, infant child #2 got picked up from facility with authorized representative.
- At 10:15am Licensee Jasmine McDaniel arrived at the facility and LPA discussed purpose of today's visit. Licensee stated C1 and C2 are overnight children, are suppose to be picked up by 9am and parents picked up late.
- At 10:35am LPA observed (4) infants and (2) children with (3) assistant staff, making census 6 at this time.
Per California Code of Regulations, Title 22, Division 12, (1) deficiency (Type B) was cited during today's visit. Citation on LIC809-D page.
A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).
The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit.
A Notice of Site Visit was given and must be posted for 30 days and appeal rights were provided. An exit interview was conducted with Staff Brandi Bowman
