25 | Licensing Program Analyst (LPA) Alicia Mooberry conducted an unannounced Case Management visit for the purpose of obtaining signature for an amended report. LPA arrived to the facility at 9:30 and met with Brandi Bowman, Staff, who granted LPA entry to the facility. Upon arrival LPA observed a total of 8 children in care: Five (5) infants under age of 2 and three (3) children age 2. There was one additional Staff. The facility is out of compliance over the ration of the license. At 9:38am, infant child #1 and #2 got picked up from facility with authorized representative.
- At 9:43 am, infant child #1 and #2 got picked up from facility by authorized representative.
- At 9:50am Licensee Jasmine McDaniel arrived at the facility and LPA discussed purpose of today's visit. Licensee stated C1 and C2 are overnight children, are suppose to be picked up by 9am and parents picked up late.
Per California Code of Regulations, Title 22, Division 12, (1) deficiency (Type A) was cited during today's visit. Citation on LIC809-D page. This is a repeat violation. A Civil penalty of $250 is assessed today
A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year).
The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit.
A Notice of Site Visit was given and must be posted for 30 days and appeal rights were provided. An exit interview was conducted with Licensee Jasmine McDaniel.
