25 | Licensing Program Analyst (LPA) Ashley Calderon conducted an unannounced Case Management visit for the purpose of delivering report from an office visit conducted by the Department. LPA arrived to the facility at 12:30pm and met with Staff Alexis Lloyd who granted LPA entrance to the facility.
Upon arrival LPA observed a total of 7 children in care. (4) infants under age of 2 and (3) children age 2 and there were two Staff Assistants.
The following was observed:
- At 12:25 pm, Infants: Child #1 (C1),Child #2 (C2), Child #3 (C3), and Child #4 (C4) were observed with sleeping with blankets in their play yards, deficiency cited. Staff present at 12:37pm removed blankets from children's play yards.
- At approximately 12:45pm Licensee Jasmine McDaniel arrived at the facility.
- LPA observed Child #5 (C5) with a bobby pillow and a blanket. LPA conducted records review and child recorded indicated child was 2 years and 6 months. Per staff A.Lloyd and Licensee Jasmine McDaniel child is transitioning to a cot but is having a rough time. LPA consulted with Licensee any children in play yard and cots per regulations shall be free of loose articles and objects. LPA provided 102425 Infant Safe Sleep Title 22 Regulations.
During visit Licensee stated is interested in Technical Support Program (TSP).
Based on today’s visit, Per California Code of Regulations, Title 22, Per Regulations Title 221 deficiency cited on today's visit provided on LIC 809-D.A notice of site visit was given and must remain posted for 30 days and appeal rights provided. Exit interview conducted and report was reviewed with the Licensee Jasmine McDaniel. |
