On May 10, 2024, at 12:30 pm, Licensing Program Analysts (LPAs) A. Wallin and A. Moobery, conducted an unannounced Annual Required Inspection for the infant center license. During today’s inspection, LPA met with director, Katrina Williams, and toured the facility indoors and outdoors. Days and hours of operation are Monday through Friday from 6:00 AM – 6:00 PM. This facility consists of 2 classrooms and outdoor play area. During inspection, LPAs observed 11 infants and three staff present. Per director, 12 infants enrolled at this time. Capacity and limitations as specified on the license are being maintained.

The following was observed during the tour of the facility: furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. Carbon monoxide and smoke detectors were observed and are operable. LPAs observed a barricaded wall heather located in the classroom. Children have their own cubby to store their belongings. Linens are washed by the facility weekly. The facility has sufficient age-appropriate furniture, and equipment including cribs, cots or mats, changing tables and feeding tables. General sanitation was observed. During inspection, LPAs observed infant cribs arranged without a proper walkway. Director addresses issue immediately and rearranged cribs to make accessible walkway for staff. A technical violation citation issued.

Drinking water via Arrowhead jug water dispenser located in kitchen. Per PIN 21-21-CCP lead testing to be conducted for all equipment used for drinking consumption. Per staff water tested is used for drinking and food preparation. LPA observed negative lead testing results conducted 9/26/23. Disinfectants, cleaning solutions, medication and other items that are dangerous to children, were inaccessible to children. Menus are posted in the classroom one week in advance. All kitchen, food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair.

Outdoor playground equipment is in a safe condition, free of sharp, loose or pointed parts. During inspection, LPAs observed fence located on right side of infant playground open and unable to close due to orange electrical cord. Director addressed immediately and closed gate. A technical violation citation issued. During inspection, LPAs observed cushion padding under the outdoor swing set coming undone causing a potential tripping hazard. A type B citation issued. There is adequate shade in the play yard. LPAs advised that no children shall be left without the supervision of a teacher at any time. There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

LPAs reviewed 5 infant’s file and observed. Per children record review, LPAs observed files are incomplete either missing parent signatures or documents. A type B citation issued. Per children record review, LPAs observed one infant’s file missing LIC9227 Infant safe sleep plan. A type B citation issued. Per children record review, LPA observed three out of five files missing infant needs and services plan. A type B citation issued.

Per staff record review, LPA observed the following documentation maintained and updated: LIC-501: Personnel Record, LIC 508- Criminal Record Statement, LIC 9052- Employee Rights, LIC 503- Health Screening, Proof of immunization against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse. Per staff record review, LPAs observed 2 staff and director without EMSA approved 1ST aid/CPR certificate. LPAs observed one staff with CPR and Pediatric First Aid with EMSA approved sticker. Per staff record review, LPA observed one teacher missing infant units. A type B citation issued. The name of the childcare center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day. LPAs reviewed the following documentation to be kept in facility: facility license, LIC610, PUB 393, daily schedule, PUB 269, fire drills and monthly menus.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP
LPA referred licensee to the Department website for lead: Lead Toxicity Prevention and Water Testing Information.

LPA discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility does not provides Incidental Medical Services – IMS. Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
