On Thursday, March 2, 2023 at 12:43 p.m., Licensing Program Analysts (LPA’s) Mayra Rivera and Rosaura Valenzuela conducted an unannounced annual inspection and met with director Vilma Medina who guided LPA’s Rivera and Valenzuela on a tour of the facility. During arrival, LPA Rivera observed 11 children napping and 4 staff present.

This is a preschool program with a toddler component. This program serves children ages 18 months to 5 years and operates Monday to Friday from 7:00 a.m.to 5:00 p.m.



All areas identified on the facility sketch were toured and inspected both indoors and outdoors. Sign in and out takes place at the entrance of the classrooms. LPA Rivera observed 11 signatures and 11 children present during the visit.

At approximately 12:49 p.m., LPA’s Rivera and Valenzuela entered toddler classroom to conduct an inspection. Per director Vilma, the classroom has not been utilized due to no staff or toddlers enrollment. The facility is having a hard time getting toddler children. LPA observed age appropriate furniture and equipment in good condition, free of sharp, not loose, or pointed parts and age appropriate toys. LPA observed a trash bin with a tight-fitting lid.

At approximately 12:58 p.m., LPA’s Rivera and Valenzuela entered preschool classroom to conduct an inspection. LPA Rivera observed age appropriate furniture and equipment in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed cubbies with children personal items. LPA observed the cleaning solutions stored inside the top cabinet making it inaccessible for children to reach. LPA observed a trash bin with a tight-fitting lid.

Heating lighting and ventilation were evaluated by LPA Rivera . LPA observed central AC/heater and vents located on the ceiling wall. LPA observed the thermostat located in the toddlers classroom and the current room temperature to be at 73 degrees LPA observed the fire extinguishers located inside the classrooms and the valve on the required 2A10BC fire extinguisher indicating fully charged and serviced on 7/11/2022. The fire extinguisher located inside the kitchen, LPA observed the valve on the required 2A10BC fire extinguisher indicating fully charged and serviced on 7/14/2022 Smoke detector and fire alarm located in classrooms on the top ceiling and connected to the fire alarm system. LPA observed a carbon monoxide in the preschool and toddler classroom. At approximately 1:04 p.m., LPA tested the carbon monoxide detector located in the toddler’s classroom. LPA Rivera heard the sound and the carbon monoxide detector is operable. LPA observed the first aid kits to be in the classrooms and observed the kits to be fully stocked with band aids, gauzes, adhesive bandages and antiseptics and located in the classroom’s cabinets. LPA observed the fire/earthquake log and last drill conducted on 2/13/23. For ill/isolation area children utilize the director's office. During this visit, per director Vilma the facility currently does not have children with medication or food allergies.

At approximately 1:10 p.m.,LPA Rivera observed the restrooms to be in good condition with working toilets, sinks, hand soap dispenser and paper towels. LPA observed a total of three toilets, five sinks and two diapering changing tables. Restrooms are in the classrooms. Napping equipment (cots) were observed to be stored in the staff lounge and sheets and blankets are provided by the facility. For ill/isolation area children utilize the staff restroom located across from teacher's lounge.

This program provides breakfast, lunch and p.m. snack. The meals and snacks are catered by Dignity Health California Hospital Medical Center. The meals and snacks are delivered daily. A monthly menu is available and posted in each classroom. For water drinking LPA observed disposable water cups and water pitchers in the classroom. LPA observed the kitchenette food preparation area to be clean and free of litter, rubbish, rodents, and/or any other vermin and storage containers for solid waste. The facility was observed to be free of flies, other insects and rodents.

At approximately 1:23 p.m., LPA Rivera entered the outdoor playground to conduct an inspection. LPA observed two separate yards (toddlers and preschool) and observed the equipment’s to be in safe condition, free of sharp, no loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All areas around or under high climbing equipment, are cushioned with granulated rubber material that absorbs a fall. LPA observed the tag on the preschool playground structure for ages 2-5 years. The shade sails provide adequate shade in the play yard. LPA observed the gates closed and the push bar has an alarm. The sandbox is covered with a tarp and compressed down with clips. For outdoor water drinking, LPA observed disposable water cups and water pitcher. LPA did not observe bodies of water on the premises.

At approximately 1:30 p.m., LPA Rivera observed on the bulletin wall the license, Parents Right's, Personal Rights, LIC 610 Emergency Disaster Plan, Facility Sketch, LIC 9148 Earthquake Checklist, PUB 269 Child Passenger System Poster, monthly Menu and Daily Schedule. At approximately 1:45 p.m., LPA Valenzuela reviewed the children and staff files. All individuals present have obtained a criminal record clearance or criminal record exemption. There is at least one person trained in CPR and Pediatric First Aid Vilma Medina present during this inspection. Educational background, training, and/or experience for each staff present are on file and were reviewed.

Director Vilma Medina has proof of pediatric First Aid/CPR certification (expires 12/9/2023), preventative health and safety training with the ,lead component dated 1/6/22, child abused mandated certification (AB 1207) dated 1/28/22 and immunization against pertussis, and measles, and influenza. Director has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com


Items discussed during this visit:

1. The following items are an immediate citation by Licensing: Refused Entry to a Facility or Any Part of a Facility is a violation of Section 1596.852, 1596.853 or 1597.09. Regulations 101238 (g) (2), The Presence of an Excluded Individual, Fire Clearance Violations, Accessible Bodies of Water, Accessible Firearms, Ammunition or Both.

2. Breakfast/Lunch/Snack Menus: Menus are required to be posted one week in advance where it is visible by the child's authorized representative. Menus for the past 30 days must also be available upon request.
3. Current Children’s Roster: Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.
4. LIC 311A - Records to be Maintained at the Facility: Child Care Centers was provided to the applicant during this visit. LPAs advised that forms, regulations and quarterly updates can be accessed on the Child Care Licensing website at: www.ccld.ca.gov.
5.Pediatric First Aid and CPR: At least one person trained in Pediatric First Aid and CPR must be present.
6. Designated Staff: The name of the childcare center director or fully qualified teacher(s) designated to act in the director's absence must be on file.
7. Qualifications: Educational background, training, and/or experience for each staff present must be available for review.
8. Immunization Requirements: Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director Vilma Medina was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with director Vilma Medina and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed director Vilma Medina of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

At this time, the licensee is in compliance with California Title 22 Regulations. Therefore, there are no citations being issued today A notice of site visit was given and must remain posted for 30 days.
