25 | Licensing program analyst (LPA) Randy Derraco conducted an unannounced case management visit to follow up an Unusual Incident Report on 04/29/22. LPA arrived at the facility at 12:15 PM as was met by Director Donna Belvin. Director accompanied LPA on tour of the facility. LPA observed 24 napping children in care and 7 adults.
The child care center consists of 3 classrooms and one infant room. LPA observed the the classrooms to be clean a free of defects. Bathrooms designated for children in care were observed to be sanitary. Required postings were observed throughout the center. The outdoor play area was observed to have child sized tables, child sized chairs, and outdoor play equipment. Perimeter fencing was observed in the outside play area. A green mesh cover was observed to cover the fencing leading the to parking lot. Director advised that a section of the fence has been covered twice with the mesh in order to cover gaps. Per director, the second mesh cover was installed the day of the incident. Children's records were reviewed for completeness. Staff records were also reviewed during inspection.
Per Director, S2 was with the children when incident occurred. S2 states her and her aide were watching children during recess. She also states that children in room 4 were the only ones out in the play yard at the time of the incident. Per record review, there were 16 children in attendance for room 4 on 04/15/22. S2 states she noticed child had slipped half way through the fence and ran over and held her arm so she would not go all the way through. She then signaled for her aide to go outside of the fence and bring her back inside the play yard. S2 states the incident lasted about a minute and a half. Per S2, teachers watching children are now required to change positions every 20 minutes.
No deficiencies will be cited during todays inspection. A notice of site visit must be posted for 30 days. Failure to comply with posting requirements will result in an immediate civil penalty of $100. An exit interview was conducted, appeal rights provided, and report was reviewed with Director, Donna Belvin. |
