On 12/15/21 at 9:45 AM Licensing Program Analyst (LPA) Jeanette Estrada conducted an announced pre-licensing inspection at 9308 Deeble St South Gate, CA 90280. The purpose of this inspection is to inspect and evaluate the facility for an initial license. LPA Estrada met with Applicant, Richard Sanchez. Applicant is requesting a small license. Operating hours will be Monday to Friday 6:00AM to 6:00PM and care will be provided for children ages 2 to 12 years. All residents living in the home were discussed and verified.

This is a one story home which consists of 3 bedrooms, 1 restroom, living room, kitchen, fenced front yard and detached garage (off limits). Applicant Richard Sanchez guided LPA on a tour of the facility indoors and outdoors. All areas identified on the facility sketch were inspected, including but not limited to, off limit areas.


Per applicant areas off limits to children and parents are: 1 bedroom (back bedroom), outdoor area behind the home and the garage. LPA observed the bedroom to be locked. The outdoor area behind the home and the garage are separated by a gate on the side driveway. There is no access to the garage through the home. There is another home with a separate address which is not licensed located in this part of the off limits outdoor area. The off limits bedroom has direct access to this outdoor area. This bedroom will be kept locked during operating hours. The applicant understands that off limit areas are to be inaccessible to children at all times and that Licensing staff may have access to off-limit areas during inspection visits if necessary.
Per Applicant, the following areas are accessible to children in care: The living room, kitchen, bathroom and two bedrooms. The daycare areas (living room and two bedrooms) that will be used by children were observed for safety, comfort, cleanliness, telephone service (cell phone and landline), ventilation and heating. For ventilation, Applicant states windows will be kept open and there are ceiling fans in the bedrooms. There is no wall heater or central heating. Applicant will provide heat via a portable heater. LPA advised applicant to keep heater out of reach of children.

Detergents, cleaning compounds, sharp objects and hazardous items that can pose a danger to children are inaccessible to children in care. Knives and sharp objects are kept in a upper cabinet in the kitchen, currently there are no medicines in the home but applicant stated medicines will be kept in an upper cabinet out of reach for children, cleaning compounds are kept under the kitchen sink and bathroom sink in latched cabinets. There is a laundry area that has been made inaccessible with a safety gate. Laundry detergent is kept in this area in an upper cabinet. Per Applicant, the children will be escorted to the restroom depending on age of the children. Personal Hygiene items in the bathroom are inaccessible to children. Per the applicant, there are no poisons other than cleaning compounds in the home. LPA advised that if any poisons are kept in the home at a later date to keep them in a locked cabinet or container.

LPA observed a combination smoke detector and carbon monoxide detector in the kitchen. LPA observed Applicant push the test button which indicated it is operable. LPA observed a 3A40BC fire extinguisher which indicates full and indicates it was serviced on 12/2021. Fire extinguisher is kept in the hallway. The home has electrical outlet covers throughout the day care area and maintains an emergency kit. A first aid kit for everyday use was not available. LPA did observe age appropriate materials and toys available for children. LPA observed napping cots. Per the Applicant, children will nap in the living room and in the two bedrooms. Applicant stated there are no firearms or weapons on the premises. Per the applicant there are no bodies of water on the premises. LPA did not observe any bodies of water on the premises.

Per the Applicant, if a child becomes ill, they will be taken to the living room area and the other children will be separated in the bedrooms. The kitchen is on limits to children but only during meal times. Per Applicant, food will be provided for children in care and children will be supervised while eating in the kitchen. LPA observed one high chair and a small table for up to 4 children. LPA advised Applicant that high chairs are to only be used for eating during mealtimes. Applicant was advised that if food is brought from the children’s homes, the container shall be labeled with the child’s name and properly stored or refrigerated.

The on limits outdoor area that will be used is the fenced front yard and side driveway. The fence opens up to Deeble St and it will be kept locked during operating hours. This outdoor play area is accessed through the laundry room and through the front door. Children will access the outdoor play area through the front door only. The door in the laundry room will be kept locked. There is grass throughout the yard. Applicant was advised to pull sticks of a plant that were sticking out in the front yard. Per Applicant, children will be supervised while playing outside.

Applicant has completed the required Health and Safety Training, Nutrition Training, Lead training and Pediatric First Aid and CPR which expires 04/25/2022. Applicant also completed the required Mandated Reporter Training which expires 11/8/2022.
Covid -19 Information: LPA and Applicant discussed COVID-19 procedures. Per Applicant, masks will be required for children ages 2 and older and all adults. Parents will drop off and pick up children at the front door. Applicant will take the child's temperature and will do a health screening before entering. Applicant has additional masks for children if needed and other hygiene supplies; Hand Sanitizer, soap. Applicant stated they will be disinfecting toys throughout the day and entire daycare area will be cleaned and disinfected after children have left for the day. Napping mats will be spread out throughout the living room and bedrooms to allow for social distancing.

Safe Sleep: Applicant states that care will not be provided for children under 2 at the moment. LPA advised if applicant decides to care for infants to sleep infants where they can be directly supervised at all times and advised against sleeping infants in a separate room. Applicant states that if enrolled, infants under 12 months will sleep in the living room or in bedroom in a crib or play pen and over 12 months will sleep on a mat if able to. LPA discussed the following with the Applicant: Cribs or play yard shall not hinder the entrance or exit from the sleeping space, mattresses shall be firm and covered with a fitted sheet that overlaps the underside so it cannot be dislodged. Infants shall not be swaddled while in care. LPA advised the Licensee that infants shall be placed on their backs for sleeping and shall be supervised. All infants shall be checked on every 15 minutes and the time of each 15-minute check shall be documented with child’s name and date. The LIC 9227 Individual Infant Sleeping Plan shall be completed for each infant up to 12 months of age. A copy of the LIC 9227 was provided to Applicant. LPA did not observe any children left in parked vehicles. Car seats shall only be used for transportation. LPA did not observe any children sleeping in car seats. LPA discussed and provided the applicant with a copy of the Infant Safe Sleep Regulation 102425 and the Infant Sleep Plan (LIC 9227).

The following was discussed with the applicant:

· Individuals who are 18 years of age or older living in the home must obtain a criminal record clearance. Individuals within one month of their 18th birthday must be fingerprinted immediately. Civil Penalties will be assessed if not in compliance.


· In the absence of the licensee a qualified adult must be present supervising the children; a qualified adult is an individual who has a valid and current Pediatric First Aid and CPR training, Immunizations (TDAP, MMR, Influenza), mandated reporter training and a valid criminal record clearance associated to the facility license.
Annual fees must be paid promptly and by the due date or a late fee shall be assessed and/or the License may be terminated. The fire extinguisher type 2A-10BC or bigger must be serviced annually or as often as necessary. Smoke and carbon monoxide detectors should be checked and batteries replaced as needed.
· Reporting Requirements: Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your home.

· Reporting Requirements: Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing. Unusual Incident report email: MPSWIncidentreports@DSS.CA.GOV

· Fire and safety drills must be performed every six months and documented for review by the Department.


· Smoking is prohibited in a family child care home.
Children and Staff records must be maintained and updated as needed and must be available for review by the Department.
· No infant walkers, No Johnny jumpers, No saucer chairs, No trampolines and any other item that falls into that category are not permitted in the facility.
· Inspection Authority: All adults living and working in the home shall be made of aware of the Department’s right to inspection the home, which includes, but is not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation.
· The facility license number must be on all advertisements, publications or announcements with the intent to attract clients.

· Isolation for Ill children: When a child is ill he/she shall be separated from other children (reference 102417(e) Operation of a Family Child Care Home). Isolation area will be in the living room.
· Liability Insurance was discussed; LPA advised applicant to review Title 22 Regulation 102417(m)(1) for additional information. At this time, the Applicant does not have Liability insurance.

Immunization Requirement: H&S 1597.622: Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. The Applicant has submitted proof of immunization vaccines. Mandated Reporter Training: H&S 1596.8662: Beginning January 1, 2018, all licensed providers, applicants, directors and employees to complete training as specified on mandated reporter duties. Training is available at: www.mandatedreporterca.com . Incidental Medical Services (IMS) policy was discussed.   • Install lock on back gate

A small family child care license will be granted upon receipt of proof of corrections for the above and Manager approval. Once licensed, the Applicant is required to comply with the terms and limitations stated on the license. Exit interview was conducted with Applicant. A copy of this report was reviewed during the inspection. LPA provided a copy of the report and the appeal rights to the Licensee/Applicant
