25 | Licensing Program Analysts (LPAs) F. White and K. Dunlap arrived at the above facility for the purpose of an unannounced Case Management - Incident inspection. LPA White introduced K. Dunlap, and announced the purpose of the visit and was granted entry into the facility by Director, Claudia Ceballos. There are 81 children enrolled, and 79 children present at the time of the inspection. There are 21 staff members present, and all have criminal background clearance.
An Unusual Incident Report (UIR) was received to the department on June 13, 2024 regarding a parent's refusal to communicate with facility staff about the escalating behavior of Child #1, or accept the invitation of director to observe the child's behavior in the classroom. LPA White reviewed the child's file and saw evidence of multiple attempts to contact the parent regarding the child's behavior. This evidence includes photos of the child mid action. There is also a statement from the mother stating that she did not want to be contacted about her daughter's behavior anymore. Based on the child's file, the parent was offered the ability to observe during the class time, but she refused.
LPA White conducted observations, interviews with staff and record review looking for a link between the escalating behavior from March to June 2024. During the interviews with staff that worked the closest with Child #1, staff shared the many opportunities they gave Child #1 to correct her behavior. Director states, that she would allow child #1 to walk round with her when behavior was out of control.
Director Claudia Ceballos provided LPA White with a copy of the Parent Handbooks and Admission Agreement initialed and dated by Child 1's parent. Admission Agreement states, "One-to-one Care: LPL cannot offer one-on-one care for students. Each student will be evaluated in their first 30 days of care. If a student has special needs or is unable to follow class activities for 50% of the time a meeting will be held with parents/guardians to discuss further action. LPA White observed the parent's initials on the Admission Agreement in the file.
