On 6/29/22 at Licensing Program Analyst (LPA) Jeanette Estrada conducted an unannounced case management inspection at 1021 S. Bonnie Beach Pl Los Angeles CA 90023. The purpose of this inspection is to inspect and evaluate the facility for an increase in capacity. The facility is currently licensed as a small. LPA Estrada met with Licensee, Maxima Zuniga. Operating hours will be Monday to Sunday 12AM to 11:59PM and care will be provided for children ages 0 to 12 years. All residents living in the home were discussed and verified. An approved fire clearance was received on 6/13/22.

This is a 3 bedroom/1 bathroom home with a front play area, living room, kitchen and back outdoor area. There are two additional homes that sit on the same premises. The back outdoor area of the home pertains to one of the additional homes. The back entrance of the home is maintained locked. The additional homes have separate addresses.

Areas on limits to children: Front outdoor play area, 1 bedroom, bathroom and living room. The living room is used as the main play area. The front yard is fenced off from the rest of the driveway. The other residents of the other homes have access to the driveway.

Off Limit areas: 2 bedrooms and kitchen. One of the bedrooms is accessed through the kitchen. This bedroom is kept locked. The Licensee has installed a safety gate blocking access from the living room to the kitchen. The other off limits bedroom is accessed through the living room. This bedroom also remains locked during operating hours and it has a safety knob installed as well. This bedroom is also accessed through the on limits bathroom. The applicant has installed a safety door knob on the door in the bathroom. .

Areas accessible to children were inspected the for safety, comfort, cleanliness, ventilation and working phone (cell phone). Children nap and eat in the living room. LPA observed children napping on mats. Per licensee bed sheets are sent home weekly or as needed to be laundered. LPA observed an A/C unit in the living room. A portable heater will be used for heating and will be kept inaccessible to children. The bathroom children use is in between the on limits bedroom and one of the off limits bedrooms.

Children enter the restroom through the on limits bedroom only. Per Licensee, children are escorted to the restroom. LPA did not observe any hazards in the restroom. Personal hygiene items were kept in a latched cabinet under the sink. The outdoor play area is in the front of the home. Per Licensee, children are supervised when playing outside. LPA observed age appropriate furniture, toys and materials to be in good condition.
Detergents, cleaning compounds, sharp objects and hazardous items that can pose a danger to children are inaccessible to children in care. Knives, sharp objects and medicines are kept in a latched upper cabinet in the kitchen, cleaning compounds are kept in a latched cabinet under the kitchen sink. The stove has safety knobs installed. Per the applicant, there are no poisons other than cleaning compounds in the home. LPA advised that if any poisons are kept in the home at a later date to keep them in a locked cabinet or container.
There is an operable smoke detector and a carbon monoxide detector in living room. LPA observed a 2A10BC fire extinguisher which indicates full and proof of date of purchase is 2/23/22. The fire extinguisher is installed on the living room wall next to the kitchen. The home has electrical outlet covers throughout the day care area and maintains a first aid kit. LPA did observe age appropriate materials and toys available for children. Per Licensee there are no firearms or weapons in the facility. Per the applicant there are no bodies of water on the premises. LPA did not observe any bodies of water on the premises.
Per the Licensee, the on limits bedroom is used as the isolation room incase a child becomes ill and is waiting to be picked up. Food is provided to children in care. Licensee was advised that if food is brought from the children’s homes, the container shall be labeled with the child’s name and properly stored or refrigerated.LPA advised Licensee that high chairs are to only be used for eating during mealtimes.
LPA observed the required postings: Facility License, PUB 394 Parent's Rights Poster, LIC 9148 Earthquake preparedness poster . LPA observed LIC 610 Emergency Disaster Plan, LIC 999 Facility Sketch, Emergency drill log with last drill conducted on 6/7/22. Licensee had an updated facility roster available.
Licensee has current pediatric CPR/First Aid which expires 2/5/2024, Mandated Reporter training which expires 9/9/2023 and required Health & Safety plus one hour nutrition and lead component completed. Licensee has required immunization vaccines on file for Measles and pertussis and a declination for the influenza vaccine.
Infant Care: Currently Licensee does not have any infants enrolled. LPA observed that there is one play yard available. LPA discussed the following with the Licensee. Napping equipment should not block entrances or exits. Infant mattresses should be firm with tightly fitted sheets. LPA discussed that each infant enrolled must have their own crib/ napping mat and bedding.

LPA and Licensee discussed the Safe sleep regulation, including LIC 9227 Infant Sleep Plan for infants under 12 months, 15-minute sleep check documentation for infants 0-24 months, will provide PIN 20-24-CCP.
Overnight Care: There is no overnight care provided at the moment. LPA discussed the following: Licensee is aware that they must remain wake while children are awake. If children sleep in separate area from licensee, the door must remain open. If licensee cannot hear children when they wake up, video or audio device can be used.
Incidental Medical Services (IMS):
Incidental Medical Services (IMS) policy was discussed. Licensee does provide Incidental Medical Services. Licensee has submitted an IMS plan and is kept on file. Medication is kept in the off limits kitchen, inaccessible to children. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. When any IMS is provided, a plan for providing IMS must be submitted to the Department. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
LPA discussed the safe sleep regulations with Licensee and discussed the Child Care Licensing Safe Sleep webpage athttps://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleephttps://www.cdss.ca.gov/inforesources/child-care-licensing/ public -information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.
No deficiencies were cited today.
A large family child care licensee will be granted upon LPM approval. Once licensed, the applicant is required to comply with the terms and limitations stated on the license. Exit interview conducted and report was reviewed with the Licensee, Maxima Zuniga. A notice of site visit was given and must remain posted for 30 days.
