On Thursday, August 11, 2022 at 10:45 a.m. Licensing Program Analyst (LPA) Mayra Rivera conducted a pre licensing inspection and met with Vice President Anna Lopez, Associate Director of Education Amanda Velasquez, Facilities Manager Jose Martinez and Quality Assurance Specialist Reza Khosrowabadi who guided LPA Rivera on a tour of the facility.

This is a proposed infant program with toddler component and will serve children ages 12 months to 36 months. This program will operate Monday - Friday 7:45 AM – 3:15 PM. Sign in/out and health check screenings will take place at the front entrance of each classroom. All areas identified on the facility sketch were toured both indoors and outdoors; measurements were taken. The applicant is seeking to provide care for 16 infants and 64 toddlers. A fire clearance has been granted as of July 7, 2022 for 16 infants and 64 toddlers.

At approximately 10:55 a.m., LPA Rivera entered the outdoor playgrounds to conduct an inspection and measurements. LPA observed two yards (infant and toddlers). LPA observed the yards to be fenced all around and with two push doors. LPA also observe two emergency push doors with installed alarms notifying when the doors open.

In the infant area, LPA observed the surface of the outdoor activity space maintained in a safe condition and is free of hazards. LPA observed soft cushioned padding around and under the playground equipment and the cushioned padding absorbs a fall. LPA observed age appropriate play equipment and to be in safe condition, free of sharp, no loose or pointed parts. For outdoor drinking water, the facility is providing a water beverage jug cooler, sippy and disposable cups. For adequate shade, LPA observed a patio ez up canopy and three of the ez up legs are zipped tied to the fence making it unmovable by the wind.

In the toddler’s area, LPA observed the surface of the outdoor activity space maintained in a safe condition and is free of hazards. LPA observed artificial turf around and underneath the playground equipment. The artificial turf is cushioned with granulated rubber 3” in depth and absorbs a fall. LPA observed age appropriate play equipment and to be in safe condition, free of sharp, no loose or pointed parts. For outdoor drinking water, the facility is providing a water beverage jug cooler, sippy and disposable cups. For adequate shade, LPA observed a table umbrella and a patio ez up canopy and three of the ez up legs are zipped tied to the fence making it unmovable by the wind.

At approximately 11:25 a.m. LPA entered classroom 5B (12-18 months) to conduct an inspection and measurements. LPA Rivera observed a sink, age appropriate furniture and equipment to be in good condition, free of sharp, not loose, or pointed parts and age appropriate toys accessible to children. LPA observed a diapering table with arm’s reach of a sink and maintained in good repair and safe condition. LPA observed a bathroom and kitchenette area in between classrooms 5B and 5A. LPA observed 2 toilets and 2 sinks. LPA Rivera observed the bathrooms located inside the classrooms to be in good condition with working toilets, and sinks. LPA observed hand soap dispenser with soap and the paper towel dispensers with paper towels and a step trash bin with a tight-fitting lid. LPA observed a total of 10 toilets and 10 sinks. Ill/isolation area children will utilize the staff bathroom located in front of classroom 1A.

Heating lighting and ventilation were evaluated by LPA Rivera. LPA observed central AC/heater and vents located on the ceiling walls and the thermostats in classrooms 1A, 2A, 3A, A4 and A5. LPA observed 10 fire extinguishers. The fire extinguishers are located inside the classrooms. LPA observed the valves on the required 2A10BC fire extinguishers indicating fully charged. The service tag in classroom 1A dated 8/17/21, 1B dated 8/16/21, 2A dated 8/12/21, 2B dated 8/17/21, 3A dated 8/24/21, 3B dated 8/24/21, 4A dated 8/12/21, 4B 8/12/21, 5A 8/21/21 and 5B 8/21/21. LPA Rivera did not test the alarms as they are wired to the pull-down fire alarm system. Each classroom is equipped including the yard with first aid kits and LPA observed the first aid kits to be fully stocked with bandages, adhesives, antiseptic, gloves and scissors.

For napping equipment, LPA observed cots. The cots are stored in the kitchenette area. Facility will be providing the sheets to cover the cots and parents will be providing the blankets and if needed the facility will also be providing the blankets. The facility will be washing the sheet covers and blankets on site. For ill/isolation area children will utilize the manager’s office and a cot will be available for children use to rest if needed.

This program provides breakfast, lunch and p.m. snacks. The meals will be provided by Lynwood Unified School District.. LPA observed the menus posted inside the classrooms and outside the bulleting board. For water drinking, LPA observe water beverage jug cooler and the facility will provide sippy and disposable cups. LPA observed the kitchenette area to be clean and free of litter, rubbish, rodents, and/or any other vermin. LPA observed storage containers for solid waste, including moveable bins with tight-fitting covers that are kept on, and in good repair. LPA advised any food or solid waste need to be stored in a container with tight-fitting lid cover. The facility was observed to be free of flies, other insects and rodents.

Facility will be administrating medication. Procedure in place is the medication is stored in a Ziploc bag with child’s name, picture, consent form and medication. The medication that requires to be stored in the refrigerator, it will be stored inside a medicine box and in the kitchenette refrigerators. The Ziploc bag will be stored inside the emergency backpack which are located inside the classrooms. The backpacks are stored hanging high up on the wall making it inaccessible for children to reach. The health team comes in monthly to review the medication, and if the medication is date is near expiration date, the parents will be notified to provide an updated medication.


If the medication has expired parent's are notified and the child will be temporarily excluded until the updated medication is received..

LPA observed the documents posted on the bulletin wall located near the main gate entrance. LPA observed LIC 613A Personal Rights, Menu, PUB Child Passenger Restraint System Poster, Daily Schedule, LIC 610 Emergency Disaster Plan, Lic 9149 Earthquake Preparedness Checklist, and PUB 393 Parent's Rights Poster.

Director Martha Lopez has the incorrect Adult Pediatric First Aid/ CPR certification, has immunization against pertussis, and measles, influenza declination and Child Abused mandated certification (AB 1207) dated 5/20/22. Vice President Anna Lopez has been informed mandated reporter training must be completed every 2 years, and is available at www.mandatedreporterca.com

Criminal Record Statement


Vice President Anna Lopez was informed that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Based on today's inspection, measurements and availably of toilets and sinks the facility can accommodate the requested capacity of 16 infants and 64 toddlers. Vice President Anna Lopez has been informed; Director Martha Lopez needs the required Pediatric First Aid/ CPR certification. The plan of correction is obtaining the correct Pediatric First Aid/ CPR certification for Director Martha Lopez and due by September 12, 2022.

Exit interview, report and plans of corrections were reviewed with Vice President Anna Lopez.
