Line(s) 4 &5 on Pg 1 Line(s) 28 on Pg 3 and Line(s) 4, 7, 17, 18, 21,22, 26, 29 on Page 4 have been revised (amended).On March 17, 2025, at 1:05p.m., Licensing Program Analyst (LPA) Keneisha Dunlap arrived at the above facility for the purpose of an unannounced Annual Inspection. LPA Dunlap announced the purpose of the visit and was granted entry into the facility by Licensee- Kimberly Castro Hernandez & Frank Smith. There are 5 children enrolled, and 2 children present at the time of inspection. The hours of operation are Monday- Friday from 6:00am-5:30pm. All adults in the home were discussed and background and fingerprinted cleared. License, earthquake disaster checklist (LIC9148), disaster plan, and PUB 394 posted. The Licensee does have a current disaster log drill with the date of 2/26/25. The Licensee does have current LIC 9040 (facility roster).
This is a one-story home that consists of 2 bedrooms, 1 bathroom, Living Room, Den, Dining Room, Kitchen, detached garage, front yard, and fenced backyard. Off-limit areas include 1 bedroom, kitchen, den, front yard, and detached garage. Swamp cooler, and there is a wall heater located in the den, which is off-limits.
LPA Dunlap observed the main care area in the living room a wall mounted television, child sized table, child sized chairs, changing station, Ingenuity feeding table, 3 playpens, 2 highchairs, a cubby storage locker, a leather couch, age-appropriate toys, and educational posters. Children also nap in the main care area. There is a baby gate that separates the main care area room from hallway where off- limits areas are. LPA observed all furnishings, equipment, and material to be in good condition.
LPA Dunlap observed a bedroom designated as an activity space for children in care with age-appropriate toys, art materials, dramatic play, manipulative play, reading materials, 2 chairs, and a table in the bedroom. LPA observed sleeping cots and sleeping mats stored in the closet of bedroom 1. LPA observed all furnishings, equipment, and material to be in good condition. LPA Dunlap observed the bathroom an operable toilet and sink. Cabinets and drawers have safety latches making it inaccessible to children in care. Page 1 of 4
