1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 06/14/2023 at 9:10 AM, Licensing Program Analysts (LPAs)Katrina Chicote and Angelica Wallin conducted an Announced – Prelicensing Inspection to the above facility. LPAs met with Rossana Landeros, Applicant, and LPAs were taken on a guided tour of the facility both indoors and outdoors. Adults in the home were discussed and all have criminal record clearance. The Applicant is requesting a small family child care home license. All areas identified on the facility sketch were inspected, including but not limited to, off limit areas. This is a single-story home that consists of three bedrooms, two restrooms, Dining Room, Living Room, Kitchen, detached garage, and backyard. Per Applicant, off limits areas of the home will be all three bedrooms and Bathroom 2, LPA observed there are doors to each bedroom and bathroom that Applicant states will be locked during operating hours. LPAs observed locks to be available at time of inspection. Per Applicant, Kitchen will only serve as a walkway to gain access to outdoor play area. LPAs observed Kitchen to have two entrances (from Dining Room and hallway entrance) to have doors available that can be locked during operating hours. The Applicant understands that licensing staff may have access to off-limit areas during inspection visit if necessary. Per Applicant, Main Care Areas will be the Living Room (located upon entry of the home), Dining Room (located to the left of Living Room), and Bathroom 1 (located down the hallway to the left). Areas that will be used by children were inspected for safety, comfort, cleanliness, telephone service (cell phone), ventilation and heating (AC wall units). At 9:28 AM, LPAs observed a wall heater in hallway entrance by off limits Bathroom 2. Per Applicant, this wall heater is not going to be used and she will be using an electric space heater for heating. LPAs discussed that a wall heater must be made inaccessible to children or if inoperable a notice from the Gas Company must be provided. LPAs observed electric space heater operating that will be used during inspection which LPAs observed to not get hot to the touch. At 9:24 AM, LPAs observed a couch Report Continues - Page 1 of 5

and TV in Living Room and plastic boat play structure, ride along toys, and other age appropriate toys
available for children. At 9:27 AM, LPAs observed Bathroom 1 to have operable toilet and sink, adequate hand washing and toileting supplies, and general area safety and sanitation. LPAs observed a high storage cabinet in Bathroom 1 to contain additional toileting and handwashing supplies and to be free of hazards at time of inspection. The Applicant was advised that any poisons such as detergents, cleaning compounds, and medicines must be made inaccessible or stored somewhere with a key or combination lock. At 9:32 AM, LPAs observed these items stored up high in hallway closet making it inaccessible and in under cabinet sink in off limits Kitchen. LPAs discussed putting a lock on cabinet under sink to make these items inaccessible to children as, per Applicant, children will be using Kitchen as a walkway. Applicant was also advised that inaccessibility of hazards must be constantly reassessed depending on the children in care.

Per Applicant, the children will use the driveway and back yard for outdoor play which is accessible through Kitchen. At 9:35 AM, LPAs observed driveway to have concrete through out and have perimeter fencing and backyard has concrete patio area, grassy area, and adequate perimeter fencing through-out the yard. LPAs observed small plastic play structures on driveway and patio furniture in additional backyard play area. LPAs observed all trees, shrubs, and plants are maintained at time of inspection. LPAs observed detached garage in outdoor play area that was locked and a storage shed in corner of backyard grassy area that had a lock at time of inspection making it off limits to children. At 9:38 AM, LPAs observed hazards such as garden rakes, iron fencing leaning on wall, and open BBQ grill in backyard. LPAs discussed removing these hazards from backyard.

At 9:48 AM, LPAs observed the valve on the required 2A 10BC fire extinguisher indicates fully charged but with no service tag or proof of purchase receipt at time of inspection. Per Applicant, this fire extinguisher was provided by Crystal Stairs and she was not provided a receipt. LPAs advised that a proof of purchase or service within the year is required. At 9:49 AM, LPAs tested smoke detector and observed to be in operable condition. LPAs did not test Carbon Monoxide detector at time of inspection. Applicant states that she does not have a Carbon Monoxide detector at time of inspection. LPAs discussed requirement. Per Applicant, First Aid kit is kept in the Kitchen, LPA observed First Aid kit and was inventoried for necessary supplies. The Applicant has current Pediatric First Aid and CPR valid until 02/18/2025 and has completed the required Health and Safety Training, Nutrition, and Lead Training. Proof of immunization against influenza, pertussis, and measles was readily available during today’s inspection. The Applicant has taken the Mandated Reporter Training on 03/23/2023. Per Applicant, they will provide food to children in care and use Dining Room for eating. LPAs observed Dining Room to have Dining Room table with four chairs available for eating. Applicant states isolation area for sick children will be flexible depending on which area of Main Care Area is not being used.

At 9:40 AM, LPAs observed two small dogs, Maltipoo breed, kept in detached garage at time of inspection. Per Applicant, dogs will be kept separate when children are in care.

Per Applicant, there are no weapons, firearms in the home and there are no bodies of water around the premises. LPAs observation did not see any bodies of water around the premises or firearms/weapons at time of inspection.

The following was discussed with the Applicant:
· Individuals who are 18 years of age or older living in the home must obtain a criminal record clearance. Individuals within one month of their 18th birthday must be fingerprinted immediately. Civil Penalties will be assessed if not in compliance.
· In the absence of the licensee a qualified adult must be present supervising the children; a qualified adult is an individual who has a valid and current Pediatric First Aid and CPR training, Immunizations (TDAP, MMR, Influenza), mandated reporter training and a valid criminal record clearance associated to the facility license.
· Annual fees must be paid promptly and by the due date or a late fee shall be assessed and/or the License may be terminated.
· The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and carbon monoxide detectors should be checked and batteries replaced as needed.
· Reporting Requirements: Changes should be reported to the Department as soon as they occur such as construction, remodeling, telephone number changes and/or if you move from your home.
· Reporting Requirements: Any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing.
· Fire and safety drills must be performed every six months and documented for review by the Department.
· Smoking is prohibited in a family child care home.

· Children and Staff records must be maintained and updated as needed and must be available for review by the Department for a minimum of 3 years.
· No infant walkers, No Johnny jumpers, No saucer chairs, No infant sleepers and any other item that falls into that category are not permitted in the facility.
· Inspection Authority: All adults living and working in the home shall be made of aware of the Department’s right to inspection the home, which includes, but is not limited to the right to enter the home when children are being cared for, interview children and adults and review documentation.
· The facility license number must be on all advertisements, publications or announcements with the intent to attract clients.
· Isolation for Ill children: When a child is ill he/she shall be separated from other children (reference 102417(e) Operation of a Family Child Care Home).
· Liability Insurance was discussed; LPA advised applicant to review Title 22 Regulation 102417(m)(1) for additional information.
· Immunization Requirement: H&S 1597.622: Commencing September 1, 2016, a person shall not be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. The licensee and all adults working with children have proof of immunizations.
· Mandated Reporter Training: H&S 1596.8662: Beginning January 1, 2018, all licensed providers, applicants, directors and employees to complete training as specified on mandated reporter duties. Training is available at: www.mandatedreporterca.com

LPA discussed the safe sleep regulations with Applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA advised the Applicant how to access forms, regulations and quarterly updates on the Child Care Licensing website at: www.ccld.ca.gov. LPA reviewed and issued the Forms/Records to Keep in Your Family Child Care Home (LIC 311D) and provided the forms referenced.

OTHER INFORMATION AND FORMS PROVIDED:

Per Applicant, there are no dual licenses at this address.

Based on the LPAs observation, the following corrections need to be corrected prior to obtaining a small family child care license. Proof of corrections may be submitted via email by correction date. Corrections are due by 06/21/2023.

Proof of purchase or service tag for fire extinguisher within the year.
Wall heater needs to be made in accessible or, if inoperable, Gas Company letter confirming
Hazards observed in outdoor play area needs to be removed
