Licensing Program Analysts (LPAs) Susann Sanchez and Joshua Ortega conducted a annual inspection and a capacity increase inspection. LPA met with Licensee, Emily Alvarado who guided analyst on a tours of the facility. During this inspection individuals who reside in the home were discussed and noted. Licensee is requesting a large family childcare home license. Per Licensee, operating hours will be Monday through Friday from 6:30am to 5:00pm. Currently Licensee cares for children from 1 to 8 years of age, but is open to watch children birth to 13 years old.

All areas identified on the facility sketch were inspected, including but not limited to, off limit areas. This is a one-story home that consists of three bedrooms and two bathrooms. Areas that the children will use include living room, bathroom next to the bedroom hallway, kitchen and back yard (fenced). Off limit areas include two bedrooms, master bathroom, laundry room, backyard next to the master bedrooms and detached garage. Rooms are made inaccessible with a lock. Licensee understands that licensing staff may have access to off-limit areas during inspection visit if necessary.

Areas that will be used by children were inspected for safety, comfort, and cleanliness. LPA observed equipment, toys and age-appropriate materials for children in the living room area. Licensee will use their cell phone as the day care phone. LPA observed detergents and cleaning compounds that can pose a danger to children are made inaccessible in a locked cabinet under the kitchen sink.

Medication is locked in the hallway cabinet. Knives are stored in a locked kitchen cabinet. LPAs did not observe hazardous items in the kitchen. Per Licensee there are no poisons in the home. The Licensee was advised that any poisons must be locked, not only inaccessible. Children nap on mats and infants sleep in play yards the living room. Licensee has one dog (Shepard mix) and cat. Licensee stated that the dog and cat are kept in the backyard (the section that is "off limits") and in an "off limit" bedroom.

Licensee states there are no firearms or weapons stored in the home. The fire extinguisher was last serviced on 3/17/24. Smoke and carbon monoxide detectors were tested and are operable. Children bring food from home and food is labeled. Consult was provided that they shall be present in the home and ensure children are supervised at all times and if Licensee temporarily leaves the home, they must ensure a substitute adult with required training and documents supervise the children in their absence. Isolation area for sick children waiting to pick up by a parent will be in the dining room or entry way area, away from other children. Capacity and ratio was discussed with Licensee and a handout was provided.

Currently licensee cares for two infants. LPA reviewed of the new Safe sleep regulations, including the LIC 9227 Infant Sleep Plan for infants under 12 months and 15-minute sleep check documentation for infants 0-24 months. . LPA observed two play yards in the facility. LPA observed that napping equipment does not block entrances or exits. Infant mattresses were observed to be firm with tightly fitted sheets. LPA did not observe loose object, bumpers, objects hanging, or objects attached to the cribs. Each infant has their own play yard and bedding. Currently Licensee does not have a sleep log. LPAs gave Licensee a sleeping log and reminded Licensee to check and document every 15 minutes infants are sleeping. A technical violation was given.

Children’s records were reviewed and included (LIC) 282- Affidavit Regarding Liability Insurance, Immunization Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights. Licensee has completed the required Health and Safety Training, Pediatric First Aid and CPR (expires 03/17/25) and Mandated Reporter Training (expires 04/19/25). Licensee has proof and has submitted immunization against measles, pertussis and influenza on file. Licensee currently has 1 assistant. Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement of Child and Mandated Reporter Training Certificate.

Outdoor play area is located in the backyard. The backyard has 2 sections. The section to the left of the home is gated off. LPA observed that the backyard is fenced and free of hazards. There is a play structure. The play structure has an age limit of 3 to 7 years old. LPA did not observe any pool, spas or bodies of water on the premises.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, Licensee, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Currently there are no corrections to be made. LPA will submit application for a capacity increase to the department.
A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the licensee E. Alvarado.
