25 | Licensing Program Analyst (LPA) Raul Navarro and Licensing Program Manager Kaaren Chambers conducted a teams meeting on 11/14/23 at 1:00 PM. LPA and LPM met with Applicant Vanessa Ortega. The purpose of today's teams meeting was to go over the items needing attention on the child care center application. Topics covered during the meeting are listed below:
Based on my review of the above pending application a teams meeting will need to be scheduled with the applicant. The following items are in need of being addressed/corrected:
LIC 200 applicant is Vanessa Ortega – should be under the LLC Kreative Kidz (this will also require the profile to be updated).Noelia Rosas – no finger prints.LIC401 monthly operating indicates 36 when applying for 42.LIC404 Financial information – not received from financial institution. LIC500 does not indicate all noted – job description, assistant director, supervisor. LIC610 – staff multi-tasking – missing permission from relocation sites. Relocation site in parent handbook not the same as LIC610. Appears the facility is providing potty training – how will they dispose of the diapers and how many potty chairs do they have? ( one for every five being potty trained). Where is the medication being stored? Nebulizer – staff administering must be trained by the parent and LIC9166 completed. IMS plan missing. Pandemic plan missing. Parent handbook states “State law food brought from home must be store bought (pg12). Director qualifications – Vanessa Ortega is not qualified – no proof of “family child & community” missing proof of experience as a teacher – experience/reference letters missing. Declination flu – missing for Vanessa Ortega. Hours for director shift don’t match LIC500. Staff hours don’t match LIC500. Employee handbook has staff signing children in…why?. In-service-training – who’s providing the training for each subject to be covered. Why is staff being trained quarterly in 1st aid and CPR – who’s providing this training?. Pg 24 of EE handbook references Nevada requirement “not on duty if you have your cell phone” this does not apply to California
