 THIS IS AN AMENDED REPORT
On January 22, 2024, at 10:30 AM, Licensing Program Analysts (LPAs), Elka Chavez, Crystal Green and Licensing Program Manager, Monica Cuddy conducted an announced Pre-Licensing inspection for a Change of Ownership. Upon arrival, Licensing staff met with Facility Representative, Veronica Tapia, also present Ruhi Khan. Applicant, M.A.O.D, Inc., is requesting to be licensed for an Infant Program with 24 capacity (ages 12 to 24 months), a Toddler Program with a capacity of 24 (ages 18 to 36 months), and a preschool program with a capacity of 117 children (ages 2 to 6 years old). Hours of operation will be Monday through Friday from 7:00 AM to 6:00 PM.

All indoor and outdoor activity space utilized for the children was inspected today. The facility was observed operating under the license 198006265 (Montessori Children’s Academy). LPAs informed Veronica Tapia that staff are required to maintain direct visual supervision of the children at all times during indoor and outdoor activities. When medications are on site, Veronica Tapia stated that they will be in a file cabinet located in the main office. A fully equipped first aid kit is located in the kitchen. LPAs observed first aid kits are also kept in each classroom. There are operational carbon monoxide detectors on site located in each classroom. All required licensing documents were observed posted in the main entrance. Children will be signed in and out in the main entrance using a QR code for the ProCare App.

LPAs continued to tour the facility and measured all indoor and outdoor activity space. The preschool program will consist of 3 classrooms: room number 1 (binomial cube), room number 2 (bead stairs) and room 3 (knob cylinders). Total indoor activity space measured for the preschool 2166.38 square feet, which is sufficient to accommodate the requested capacity of 61 Children.

The Infant/ Toddler program will consist of 2 classrooms: Infant (pink tower) and Toddler (color tablets). The total indoor activity space measured for the infant with toddler component 3665.37 square feet, which is sufficient to accommodate the requested capacity of 48 children. LPAs observed all indoor activity space to be complete with safe, age-appropriate furniture and equipment, including tables, chairs, cubbies, napping cots/mats, bookshelves, and other activity supplies for the children. Drinking water is available in the classrooms via Brita water dispenser which are filled in the kitchen sink. LPAs observed all hazardous items to be inaccessible to children. There are no bodies of water or weapons on the property. Fire clearance was granted on December 07, 2023.

LPAs observed a total of 14 sinks and 11 toilets available for children’s use, which is sufficient for the requested capacity. There is a separate staff restroom equipped with a toilet and a sink. The isolation area for children who are ill will be the staff restroom next to the main office.

LPAs observed a changing table located within the infant designated bathroom and within the preschool designated bathroom. Each was observed to have at least 1” padding covered with moisture resistant, washable material. Sides of the changing table are at least 3” high and the changing table is within arm’s reach of a sink. Diapering sink is not used for meal preparation or dishwashing. The Infant program has 3 cribs which meet regulatory requirements. Crib area is separated from activity space via a partition. This barrier is at least four feet high, made of sound absorbing material and allows for supervision of napping children. Sleep logs are kept in the crib area. Needs and Services Plans and Sleep Plans are kept in children’s file.

The facility will provide children with an AM and PM snack. Parents are responsible for children’s lunch. The kitchen area currently includes a sink, refrigerator, freezer, stove, dishwasher, and microwave. The kitchen area and food storage areas were observed free of rodents and/or vermin. Food was observed to be properly stored separate from cleaning materials. The kitchen will be used as additional space for activities based on the Montessori curriculum and a waiver will requested. LPAs observed cleaning supplies are store in a cabinet above the kitchen sink. LPAs observed additional cleaning supplies are stored in the fire pane room.

The facility currently has a fully fenced playground area. There a 2 designated program outdoor areas; Infant and Preschool Program. At this time, the facility does not have a designated Toddler area. The applicant understand that a waiver will need to be requested allowing the Toddlers to utilize the designated Infant yard using a staggered play schedule. Fencing was observed to be white concrete wall and wrought iron and is at least four feet high.

The total square footage for the designated Infant outdoor activity space is 2,795.67 square feet, for only 37 children at one time which is sufficient for the capacity requested. LPAs observed adequate shade in the infant playground. LPAs did not observe shading in the side playground next to classroom number 4 which is to be used for activities. LPAs also observed a metal iron pole which is accessible to children.

The total square footage for the designated preschool outdoor activity space is 4592.11 square feet, for only 61 children at one time, insufficient for the number of children being requested. A playground waiver to share the playground at different times in the AM and PM due to limited square footage shall be submitted and approved prior to licensure. Shade is provided via patio cover and tress. There are sufficient outdoor age-appropriate toys and play equipment available on the playground. There is a climbing structure located on the designated preschool playground for children ages 2 to 12 years of age which is properly anchored. LPAs observed parts of the structure were not appropriate for younger children. LPAs observed the structure has a step pole with approximately 18 inches away from the platform, spiral ladder with approximately 20 inches away from the platform and a spiral pole with approximately 16 inches away from the platform. There is adequate cushioning in fall zones of climber provided by rubber matting. Drinking water is available via Brita water and disposable cups. LPAs observed all hazardous items on the playground to be inaccessible to children. Applicant Ruhi Khan was reminded that any changes to the facility must be reported to and approved by Community Care Licensing.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP).

The following corrections are needed prior to the issuance of the license:
· Applicant will need to place a barrier around the pole in the side infant yard to be used for activities.
· Applicant will need to make the openings on the lower platform of the play structure located in the preschool playground inaccessible.
· Applicant will need to make the access to the higher slide inaccessible.
· Applicant will need to place a potty ring and step stool in the staff restroom/ill isolation bathroom.
· Applicant will need to request a waiver to allow the toddlers to use the designated infant playground.

Facility representative Veronica Tapia understands that all proof of corrections must be provided to the Department within 30 days, or the application may be denied.

Facility representative, Veronica Tapia was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.”

LPAs discussed the safe sleep regulations with facility representative Veronica Tapia and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed facility representative Veronica Tapia of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility representative, Veronica Tapia was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
