1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Licensing Program Analyst (LPA) Franchesca White and Keneisha Dunlap arrived at the above facility for the purpose of an announced Pre-Licensing inspection. LPA's announced the purpose of the visit and was granted entry into the home by Applicant, Stephanie Alonso. Per applicant, hours of operation will be 7:30 a.m. to 4:30 p.m.. Applicant will serve children ages 3 to 5. This is a single story home with a living room, dining room, four bedrooms, two bathrooms, kitchen, laundry room, and front patio area. Areas of the home used as a main care area are living room, dining room, bathroom 1, and patio area. Areas off limits to children and parents are the four bedrooms bathroom 2, and laundry area. All areas of the home were inspected for cleanliness health, safety, heating, and ventilation for the children. LPA's inspected all life saving devices in the home. Carbon Monoxide detector is located in the dining room of the home and was operable at the time of inspection. Smoke detector is located on the back wall of the dining room, and was operable at the time of inspection. The 2A 10BC fire extinguisher was fully charged with a purchase date of October 4, 2023. At 9:45 a.m. LPA's observed the parent board to be mounted outside the front door of the home. Per applicant Parent board will be moved inside the entrance of the home. There is a mounted flat screen tv, and a entertainment center with a built in electric fireplace. Per applicant electric fireplace is not plugged for use. LPA White asked if the fireplace gets hot to the touch. Applicant states that she will purchase a barricade and or remove the entire tv center to an off limits area of the home. LPAs observed a kidney table with an adult chair, and 4 children seats.

10:15 a.m., LPAs observed the dining room area to have a sectional rug, bookcase with age appropriate manipulatives for the children. There is a whiteboard with shapes, ABC's charts above the board. There is a storage rack filled with learning materials for the children. Per applicant storage rack will be bolted to the wall to prevent any injuries of children in care.

At 10:30 a.m., LPA's observed kitchen to have baby gates located on both sides of the entry ways to the kitchen. LPA observed the kitchen stove to have child proof knob covers. Applicant states that all the cabinets have magnetized child proof locks making them inaccessible to children in care. Per applicant knives are kept in a cabinet above the sink, making them inaccessible to children in care.

At 10:40, LPAs observed bathroom 1 to have a sliding barn door with a latch. Applicant states that the door can be unlocked from the outside by lifting the door over the latch. LPA observed the applicant to lift the door over the latch to prove that a child can not be locked inside the bathroom at any time. Inside the bathroom LPAs observed a stepping stool, toilet, sink, and baby gate making the storage area of the bathroom inaccessible to children in care. LPA's observed cabinets to be made inaccessible with child proof latches. Bathroom 1 was observed to have hand washing items, toilet paper, and paper towels. Bathroom 1 was free of any floor hazards, and chemical are stored in off limits areas. Per applicant, items used by residence will be stored in their rooms that are off limits to children and parents.

At 10:50, LPAs observed the outdoor play area to have two bench tables, and dramatic play area, storage bins to be used as cubbies, a couch, a seesaw, and an activity rug for floor activities. Patio area perimeter is completely fenced.
LPA informed Applicant that upon final review of application she would be licensed for a large family child care with a capacity of 12 because Applicant Stephanie Alonso does not have a signed LIC 9149 Landlord Consent form.

Applicants states there are 2 pets in the home. LPAs observed two pets in the home. LPAs did not observe any bodies of water.

LPA reviewed Parent contract, and discussed best practices in receiving new children into the daycare. Further safety measures were discussed in utilizing the front yard as a play area in the future. Applicant understands if she decided to use any areas that were documented as off limits on facility sketches, she would need to notify the department, and provide new facility sketches with the needed changes.

Mandated Reporter training (1/19/2024) and CPR training (9/23/23) documents were available during inspection. LPA reviewed immunization records, and criminal clearances prior to inspection.

LPA reviewed with applicant, Stephanie Alonso LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant.

Applicant, Stephanie was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home.

A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.On this date, 05/23/2024, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Applicant, Stephanie Alonso was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on the LPA’s observation, the following correction needs to be completed prior to obtaining a large family child care license. Proof of the correction must be sent to LPA White by Thursday, May 30, 2024.

A large family child care license will be granted upon final application review by a Licensing Program Manager. Once licensed, the applicant is required to comply with the terms and limitations stated on the license.
