On November 14, 2024, at 9:00 AM, Licensing Program Analysts (LPAs) Elka Chavez, Alicia Mooberry and Joshua Ortega conducted an announced Pre-Licensing follow up inspection. The purpose of this visit was to remeasure the outdoor playground and make sure the corrections from September 20, 2024, were made. Upon arrival, LPAs met with Applicant Representatives, Carolyn Anderson and Nicole White. All indoor and outdoor activity space utilized for the children was inspected today.

The applicant made the following corrections:

· Applicant installed a fence to make the playground structure for children ages 5 to 12 inaccessible to children under age 5.

· Applicant placed shade in the outdoor area next to the climbing structure.

· Applicant placed sound alarms on the exit doors leading to South Park to alert when the doors are opened.

· Applicant purchased chairs for the capacity requested.

· Applicant purchased additional cots for the capacity requested.

· Applicant placed a potty ring and step stool in the staff restroom/ill isolation bathroom.

LPAs observed the outdoor area is enclosed by a fence to protect children and to keep them in the outdoor activity area near the climbing structure for children ages 2 to 5. LPAs observed the fence is at least

four feet high. LPAs observed the panels on the chain link fence are connected by wires. LPAs observed a panel is moved to allow children enter the outdoor area. LPAs advised facility representative that the fence shall remain in the same exact space. If they move the fence they must place it in the same exact space.
