25 | On October 28, 2025, Licensing Program Analyst- LPA Alfonso Iniguez conducted an unannounced Case Management/Incident visit at the facility. LPA Iniguez met with William Boles, Executive Director, and explained the purpose of the visit.
On October 14, 2025, the Regional Office received an Unusual Incident Report or LIC 624 stating that on 10/10/25 at approximately 7:00 pm, one of the residents (R#1) reported they had been physically assaulted by two facility staff (S#1and S#2). The facility executive director (A#1) assessed (R#1), and no signs of physical injuries were observed; also, (A#1) contacted the Culver City Police Department.
On October 28, 2025, LPA Iniguez spoke with (R#1), who stated that they do not remember when, but that at approximately 11:00 pm, they were asleep in bed. They were woken by someone that had physically assaulted them, (R#1) described their assailants as two African American facility staff females. LPA Iniguez asked (R#1) what they did to defend themselves, (R#1) stated that they were “throwing punches”. LPA Iniguez asked (R#1) what happened next, but (R#1) could not recall more details of what happened that “night”.
On October 28, 2025, LPA Iniguez spoke with the two-facility staff (S#1 and S#2) named by (R#1). The two-facility staff member mentioned to LPA Iniguez that they only work in the day shift and they have never physically assaulted (R#1) or any other resident in care.
According to the California Code of Regulations (Title 22, Division 6, Chapter 8), LPA did not observe deficiencies during this visit; therefore, no citations were issued.
An exit interview was conducted, and a copy of the Facility Evaluation Report was provided to William Boles / Executive Director.
