25 | On 09/11/25 at 12:00pm, Licensing Program Analyst (LPA) Perry Scott conducted an unannounced annual required visit to Serenity Seniors Home II. LPA met with DeAnthony Hardiman, Administrator, and the purpose of today’s visit was explained. The facility is licensed to serve four (4) non-ambulatory residents ages 60 and above and may retain two (2) in hospice. Currently, the facility has (4) residents. The residents are South Central Los Angeles Regional Center residents. None of the residents have Restricted Health Care Conditions and none are utilizing postural supports or protective devices. The facilities annual fees are current.
The facility is a single-story family home located in a residential neighborhood. The facility consists of the following: (3) bedrooms, (1) bathroom, living room, dining room, kitchen, front and backyard, and an attached garage and laundry area.
LPA conducted a records review of (4) resident records, (9) staff records, and reviewed the facility disaster plan. All resident and staff records were complete. The facility disaster plan was current and in compliance with Title 22 regulations at the time of visit. LPA reviewed (4) resident medication administration records and medication, and did not observe any discrepancies at the time of visit.
At 12:30pm, LPA and staff toured the facility. There are no bodies of water or firearm/ammunition on the premises. All resident rooms were checked. Beds and bedding were in good condition, adequate lighting provided, and adequate storage for resident’s personal belongings was observed. Walls and floors were in good repair. Bed linens, comforters, and bath towels were adequately stocked at the time of visit. Bathrooms were found to be within Title 22 regulations.
