25 | On 3/05/2026 at 08:10 a.m., Licensing Program Analyst(s) (LPA) Jewel Baptiste conducted an unannounced annual visit at Neardale Place, using the CARE tool. LPA Baptiste met with Direct Support Professional (DSP) Tracie Harding and explained the reason for the visit. Administrator Particie Allen arrived at 9:15 a.m. and assisted with the visit.
The facility is licensed to serve residents ages 60 and above. Fire clearance was approved for four (4) ambulatory and one (1) non-ambulatory, the non-ambulatory resident will be located in bedroom #4. The home is vendored through South Central Los Angeles Regional Center. The facility is a single-story home, located in a residential area. It consists of 4 resident bedrooms, 2 bathrooms, 1 office area, 1 laundry area, a kitchen, a detached garage, and a backyard.
LPA Baptiste conducted a tour of the facility with DSP and observed the following: The facility is clean and in good repair indoors and outdoors. The fire extinguisher observed in the kitchen and hallway was charged, the facility contacted fire department for there tag. The cleaning supplies were locked in the hall closet, and the sharps were locked in a kitchen drawer. Sufficient food supplies were observed, at least 2 days of perishables and 7 days of non-perishables. The four (4) bedrooms were observed with sufficient lighting, furniture, and bedding supplies. Two (2) resident bathrooms were observed in working conditions with grab bars and skid mats. The water temperature was tested at 111.1-118.4 degrees F., which is within the required 105-120 degrees F. The dining room and living room were observed with sufficient sitting areas. Smoke/Carbon monoxide detectors were tested and are in working condition. The garage contained storage supplies, water, toiletries, and extra food. The backyard was observed with a covered sitting area. PPE and toxins were locked in the hallway closet. (CONTINUED LIC 809C)
