LPA reviewed five staff files, five children’s files, and facility records. There is at least one staff member present with Pediatric First Aid/CPR training. All staff have completed Mandated Reporter Training. LPA advised that children’s files shall include Emergency Identification and Information (LIC700), Consent for Emergency Medical Treatment (LIC627), Preadmission Health History (LIC702), and a copy of the admission agreement.
LPA discussed the requirement to maintain all children’s records in the facility with D1. See LIC809-D for deficiency cited today regarding children’s records. LPA discussed the requirement for personnel records, if maintained in a central administrative location, to be readily available for review by the Department.
The facility tested its fixtures for lead after the deadline of 1/1/2023. No exceedances of lead were found in any of the tested fixtures. LPA informed D1 that a deficiency would be cited and cleared for not testing before the deadline of 1/1/2023. The facility is in compliance with lead testing requirements as of 9/27/2024. LPA advised that lead testing shall be conducted every five years.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a
maximum of 30 days per person will be assessed if this regulation is violated.
