The indoor area was arranged with toys, furniture, and educational materials for the children, without any walkers or bouncers present. Individual storage space for the children's belongings was present. Electrical outlets were observed with child safety covers. Sharp items, such as knives, were placed inside the kitchen cabinet, which was inaccessible to the children with children safety locks. An open space heater was observed to be covered with a net. The applicant confirmed the absence of firearms or weapons on the premises.
The refrigerator, stove, and dishwashing machine were observed with child safety straps. A diaper-changing station is also present in the home. The home is also observed with cots and cribs. LPA reminded the applicant that firm mattresses must be covered with size-appropriate sheets and that there must be no loose articles or materials in the crib while the child sleeps.
The front yard is enclosed by a fence. LPA observed open areas in the front yard and reminded the applicant that supervision must be always maintained in the front yard. LPA also reminded the applicant that areas around and under high climbing equipment, swings, slides, and similar equipment must be cushioned with material that absorbs falls. LPA did not observe any pools, spas, or bodies of water on site.
The applicant provided the department with evidence of her MMR and TDAP vaccinations and TB skin test. She also has a Pediatric CPR/First Aid certificate, which expires on 12/2024, and a Mandated Reporter Training certificate, which expires on 9/2026. The applicant was reminded that CPR/First Aid, and Mandated Reporter Training must be completed every two years.
Because the applicant rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).
