25 | On 2/20/2025, Licensing Program Analysts (LPAs) Miguel Herrera and Pa Kou Vue conducted an unannounced case management inspection at the facility. LPA met with Director, Jennifer Mays who accompanied LPAs during a tour of the facility both inside and outside. LPA explained the purpose for the inspection and a census was taken. During the case management visit, facility observations were completed, facility records were reviewed, and interviews were conducted.
Based on interviews and records review, LPA Herrera confirmed, that while playing in the classroom child #1 sustained an injury that required medical treatment. The child tripped and fell while playing with a sensory toy in the classroom and bumped his eyebrow on the floor. The incident occurred at approximately 4:23pm on 10/03/2024. Staff immediately treated the wound and called the child’s parent, however the facility failed to self-report the incident to the Fresno Regional Office either by telephone or fax within 24 hours of the occurrence.
Furthermore, based on interviews and records review, LPA Herrera confirmed that the facility failed to report an unusual incident that occurred on 12/26/2024, in where child #2 experienced a seizure while in care. Per interviews the seizure that child #2 sustained required medical attention. Although the facility reported the incident with child #2's parents the facility failed to report the incident to the Fresno Regional Office either by telephone or fax within 24 hours of the occurrence.
Per California Code of Regulations, Title 22, Division 12, Chapter 1, this deficiency is being cited on the attached LIC 809D. An exit interview conducted with Jeniffer Mays. A copy of this report and Appeal Rights were provided and discussed with Director Jennifer Mays. A Notice of Site Visit Form was posted to parent's board and must remain posted for 30 days.
