25 | On 12/15/22 Licensing Program Analyst (LPA) Joseph Pacheco arrived at the facility to conduct an unannounced Case Management inspection. LPA met with Licensee, Sharon Salinas, toured the facility and a census was taken. An unusual incident was reported to the Fresno Regional Office regarding an incident that occurred on 11/15/22. On that day, children were outside playing in the backyard when two children began to fuss over a toy. Child #1 was standing next to Staff #1 when this occurred. Staff #1 stepped in to resolve the issue. While Staff #1 was helping the two other children, Child #1 climbed up the steps that lead from the backyard to the sliding glass door into the home. Child #1 began to climb back down the steps when they fell and sustained a swollen and scraped upper lip and cuts to their face. Licensee and Staff #1 applied first aid to Child #1. Licensee contacted Child #1's authorized representative. Child #1 was not taken for further medical treatment and there was no injury diagnosis. Licensee immediately blocked off the steps with child safety gates and locks. During today's inspection LPA observed the area where the incident occurred. LPA observed child safety gates and locks around the steps making the area inaccessible to children in care. LPA did not observe any tripping hazards nor sharp, loose or pointed parts.
This appears to be an isolated incident and staff took appropriate measures to address the child's injury, following appropriate policies, regulations and reporting requirements.
Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, no deficiencies are cited.

This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
