32 | · Preventative Health and Safety Course with Prevention of Lead exposure certification was completed on April 29, 2023.
· Applicant completed the Mandated Reporter Training on April 18, 2023.
· SB 792 immunizations verified and on file.
· Applicant rents/leases the home, proof of landlord notification is required. LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. Applicant has not obtained a signed Property Owner/Landlord Consent form (LIC9149). Without this consent, the applicant understands that, once licensed, they can operate with a maximum capacity of 6 children. If property owner/landlord consent is obtained in the future, the applicant is advised that a new Application for a Family Child Care Home License (LIC 279) must be submitted with a change of capacity fee of $25, to increase the capacity and provide care to 8 children.
· In the front living room, LPA observed learning wall art, foam mats, wooden cubbies for storage, children size furnture, and safe toys for the children. In downstairs bedroom #1, LPA observed one play yard and an art easel. Children will nap on mats, infants will nap in play yards in downstairs bedroom. Applicant understands she is to supervise children at all times.
· LPA inspected the fenced backyard which will be off limits to the day care children. Prior to use, Applicant will contact Licensing to come clear backyard for day-care use. LPA observed a broken wooden plank, a loose wooden support beam, some protruding nails on the backyard fence, deep holes in the sodded area, and potentially hazardous items accessible to children.
· Facility has 3A40BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives are stored on top of the refrigerator and in a bottom kitchen drawer made inaccessible via safety latch. Medications are stored in the upstairs master bedroom. Cleaning compounds are stored bottom kitchen cabinet via safety latch.
· There are no bodies of water in the home or premises.
· There is a dog that is kept in off-limits areas. Applicant is advised it is her responsibility to ensure the safety of children in care at all times from the pets.
