· Applicant completed the Mandated Reporter Training for Child Care Providers on June 11, 2024.

· Applicant provided proof of control of property.
· In the play room, LPA observed children size table, chairs and furniture, a large alphabet mat, a crib, a diaper changing table, wooden cubbies for storage, and various games, toys, and books for the children. Children will nap on mats, infants will nap on crib in the play room. Applicant understands she is to supervise children at all times.
· Fenced backyard has cement, dirt, artificial turf and rubber mulch for the children. There are multiple shade sails for shade. LPA observed a four seat swing set that is anchored to the ground. LPA also observed a Burke playground with a slide attachment and blue partitions with various interactive toys and activities. Manufacturer specifications state the play equipment is suitable for children ages 2-5 years old. Both the swing set and playground are anchored to the ground. There are Lakeshore tricycles, a water table, and a sand box.
· Facility has 3A40BC fire extinguisher, smoke alarm, carbon monoxide alarm and first aid kit in place.
· Knives and medications are stored in a top kitchen cabinet made inaccessible via safety latch. Cleaning compounds are stored in a bottom kitchen cabinet made inaccessible via safety latch.
· There are no bodies of water in the home or premises.
· There are three dogs that are kept in off-limit areas in the home. Applicant is advised it is her responsibility to ensure the safety of children in care at all times from the pets.
· Applicant states there are no firearms or ammunition in the home or premises.
· Advised applicant fire drills are to be conducted once every 6 months and must be documented with date and time. A fire drill log was provided as an example.
· Applicant is advised at least one staff member with current training in pediatric first aid and pediatric CPR is to be on site at all times children are present. (CONTINUED ON 809-C) |
