32 | . LPA toured the facility. Indoor off-limit areas include the entire first floor. The child care area is located on the bottom floor, accessed from the side yard. The Licensee provided care in a safe, comfortable environment, ensuring children were treated appropriately. The home was clean, well-ventilated, and equipped with safe toys, play equipment, and materials. A fully charged 3A-40B-C fire extinguisher, purchased on 10/14/25, was present, along with working smoke and carbon monoxide detectors and an operational telephone. Hazardous items, including poisons, detergents, cleaning compounds, and medicines, were stored out of children’s reach. The home was free of defects or dangers that could harm children. The Licensee confirmed there were no firearms on the property and smoking was prohibited. The fireplace, located on the first floor is located in the off limits area and not in use during child care hours. A stairway was secured with gates at both the top and bottom to prevent child access
The outdoor areas were inspected. The back gated area and carport are off limits. The outdoor play area is fenced and contains age-appropriate toys. The Licensee was reminded to notify licensing before making any changes to the house or yard.
LPA Barnes reviewed the staff files and found one staff member missing the measles immunization record. All other required documents were present in the files. The licensee has current CPR/First Aid and Mandated Reporter certifications.. Due to the missing measles immunization, a Type B Citation was issued, as documented on the attached LIC 809D form. The LPA reminded the licensee that both CPR/First Aid and Mandated Reporter certifications must be renewed every two years to maintain compliance..
Five children’s files were reviewed and contained all required forms, including the Affidavit Regarding Liability Insurance for Family Child Care Home (LIC282). The Licensee was reminded that if transportation is provided in the future, a valid driver’s license is required, children may not be left unattended in vehicles, and appropriate car seats or booster seats must be used.
Safe Sleep Information was discussed, and licensee was reminded that an Individual Infant Sleeping Plan (LIC 9227) is a required for each infant up to 12 months old, with nap checks documented until age two. Cribs and play yards must be free of loose items, bumper pads, and hanging objects and mattresses must be firm, properly fitted, and have fitted sheets. Licensee understands that no baby walkers, bouncers, jumpers are allowed.
