Licensing Program Analyst (LPA), Elizabeth Larios conducted an unannounced complaint investigation to deliver findings. LPA met with Coordinator, Nitaya Robinson & Site Supervisor, Ashley Reyes and explained the purpose of today's visit.

The Department received a complaint alleging staff do not keep facility free of rodents. During the course of this investigation. LPA inspected classroom, conducted interviews, and obtained relevant records.

The Department found that the preponderance of evidence standard has been met based on the evidence gathered and therefore the above allegation is found to be SUBSTANTIATED.

Type B deficiency was cited today as a result of the complaint investigation and is noted on the attached page LIC 9099-D. Continue on LIC 9099-C

Based on observation classroom 10 had mouse glue traps with mice droppings. Based on information gathered mice droppings were also discovered on the floor and mice were seen in classroom. This poses a potential health and safety risk to children in care.
