1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On 04/10/2023 at 9:45 AM, Licensing Program Analyst (LPA), Susy Cervantes, met with administrator, Andrea Scott-Willis for a pre-licensing visit. LPA toured the facility both inside and out as well as measured the indoor and outdoor space. A fire clearance for 4 preschool children was received prior to today's visit. The following indoor measurements were taken today: Room 1 36.167 x 13.25 = 479.21 minus 20.04 (encumbered space) = 459.17 Room 2A 19.583 x 13.5 = 264.37 Room 2B 19.5 x 14.167 = 276.26 minus 20.75 (encumbered space) = 255.51 Room 3 34.417 x 13.167 = 453.17 *TOTAL INDOOR SPACE: 459.17 + 264.37 + 255.51 + 453.17 = 1432.22 sq. ft. divided by 35 = 40 children.* LPA informed administrator that the indoor space has space for 40 children, facility has requested 44. There are sinks 5 (75) and 3 toilet (45) available for the children that are potty training as well as 2 changing tables. Warm and cold water is available in the children's sinks. There is a separate staff bathroom that will be used by the sick child. The sick child will be isolated in the office and isolation equipment was discussed with administrator. LPA observed no fireplaces or wall heaters at the facility. The rooms have appropriate lighting. The center will be administering medication and the medication lock box is located in the office. First aid supplies are stored in each room and the office, are adequate and are inaccessible to children. Carbon monoxide detectors are operable. There is one 4A60BC fire extinguisher and two 1A10BC fire extinguishers available. Cleaning supplies are stored inaccessible to the children. The center will not be providing snacks or meals, parents will be providing snack and meals for their children. *Continues on report dated 04/10/2023_______________________________________ pg 1/3*

LPA observed solid waste container with tight-fitting lids in each classroom. There are 30 cubbies, 5 tables, 40 chairs, and 16 mats. Drinking water is available indoors and out by water dispensers.

Outdoor measurements:

UPPER PLAYGROUND (79.333 x 25.083) + (22 X 14 TRIANGLE) = 2,143.91
LOWER PLAYGROUND 55 x 36.75 = 2021.25
TOTAL PRESCHOOL OUTDOOR SPACE: 2143.91 + 2021.25 = 4165.16 sq. ft. divided by 75 = 55 children.

The playground is surrounded by appropriate fencing on three sides, the entrance side is 3'7" which is under the 4 feet requirement. There is tan bark as resilient material. LPA observed no bodies of water. Shade is provided by surrounding trees. There is a working telephone number for the center which is (831) 647-0551. No transportation is provided by the school.

Administrator was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with administrator the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.

Exit interview conducted and report was reviewed with the administrator Andrea Scott-Willis. Administrator was advised that licensure for 40 preschool children is approved pending the following:

- 20 additional mats
- Management's approval
