25 | On 07/24/2023 at 09:25 AM, Licensing Program Analyst (LPA) Susy Cervantes met with applicant, Lorena Alvarez Castillo, to conduct a change of location pre-licensing visit. Licensee is currently licensed at 665 ALVARADO COURT, SALINAS, CA 93907, license #274416009. Applicant stated that the adults living in the home are themselves, their spouse, and their daughter with two children ages 15 and 10 years old. Because the licensee rents/leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149). Applicant plans to get liability insurance and she understood that if she decides not to carry liability insurance in the future, she will have parent/authorized representative sign the Affidavit Regarding Liability Insurance for FCCH form (LIC 282).
Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
The entire home was inspected for health and safety hazards. The home has a working smoke detector, a working carbon monoxide detector, a working telephone and a 3A40BC fire extinguishers that was serviced on 04/17/2023. Certifications for CPR and First Aid expire 04/2025. Preventative Health Practices training was complete on 05/14/21. Applicant has completed Mandated Reporter training on 03/10/2022, however the general class has not been taken. Immunizations for Measles, Pertussis, and influenza are on file for applicant.
