13 | Licensing Program Analyst (LPA) Martha Jimenez-Villanueva conducted an unannounced follow up investigation today to deliver the investigation findings on the above-mentioned allegation. LPA met with Lorena Alvarez Castillo. LPA toured inside and outside the FCCH. Licensee and assistant Kimberly were providing care for 9 children, including 2 infants.
This Department gathered documents and conducted interviews with relevant individuals. During the course of the investigation, LPA gathered inconsistent stories during interviews. Based on the available evidence, it is concluded that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
No deficiencies were cited today. Exit interview was conducted and the report was reviewed with the Licensee Lorena Alvarez Castillo in Spanish and Appeal Rights were handed to her.
A notice of site visit was printed and handed to the licensee, must be posted near the entrance to the home and must remain posted for 30 days.
