25 | At 1:00pm on 5/9/2023, Licensing Program Analysts (LPAs) Matthew Gallo and Lea Habtom met with Licensee Juan Aguilar for the purpose of conducting a plan of correction visit. Licensee's two assistants were also present for the duration of the visit.
Licensee was previously cited a Type A deficiency on 4/20/2023 for operating beyond the limits of their capacity of a large license by caring for six infants and six preschool children at one time. The plan of correction dictated that the Licensee would abide by infant capacity limits and have parents sign forms acknowledging that Licensee must turn away infants if already at the capacity limit of four.
At 1:00pm, LPAs observed 12 children in care. Through interview and record review, LPA identified four as being infants and the remaining eight as being preschool children. Licensee is in ratio, therefore the plan of correction has been fulfilled. The citation of 4/20/2023 has been cleared.
In addition, licensee was previously cited a Type A deficiency on 5/1/2023 for operating beyond the capacity of their license by providing care for 17 children. The plan of correction dictated that licensee would abide by capacity limits and LPA would conduct a return visit to ensure compliance. This citation of 5/1/2023 has also cleared.
Exit interview and was conducted with licensee Juan Aguilar. LPA communicated to Licensee the seriousness of failing to correct this deficiency and the possibility of administrative actions if noncompliance should continue.
Report, appeal rights, and notice of site visit (LIC9213) were provided. Notice of site visit shall be displayed
for 30 days. |
